25 | On December 22, 2021, Licensing Program Analyst (LPA) King-Lewis conducted a Case Management inspection for the purpose to provide a health and safety check. LPA met with facility director Akemie Jones to discuss the following. On 12-03-21 at around 3:15 LPAs King-Lewis and Stepanyan arrived at the facility and no staff was available to greet LPAs. LPAs walked into the school age classroom and observed 13 school age children being care by 1 (teacher/assistant). 4 school age children were in the outdoor play area without staff supervision. LPAs questioned staff #3 where the additional staff were. Staff # 3 responded; the additional staff including the director were out on school pickups. At approximately 4:00 p.m., facility director arrived at the facility and licensee arrived shortly after.
Licensee disclosed the facility director approved 8 staff members to go on vacation without her approval causing the facility to be short staff. Licensee stated she was not aware of the staff shortage situation until LPA King called licensee.
On 12/03/21, LPAs observed the facility to be out of staff to child ratio resulting in 4 school age children to be left without adult supervision. A Type A citation is being cited. above center. Upon receipt, of a Type A deficiency licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next
