Licensing Program Analyst's (LPA) Carol Heath met with licensee, Leidy Bernasconi, who guided analyst on a tour of the facility for an unannounced required 1-year inspection. LPA observed 5 preschool age child and 5 school age. LPA observed Child Care Facility Roster. LPA observed 2 children files contained all required licensing documents. Per Licensing Information System, facility annual fees were current. The licensee is operating within proper capacity and ratios. LPA observed licensee to be present at the home and providing adequate care and supervision. Hours of operation are Monday through Friday, 7:30 AM to 4:30 PM. This is a two story 5 bedroom, 3 bathroom home with kitchen/dining, formal dining room, living room, den, family room (child care), laundry and garage. The garage is used for storage only and no childcare activities are conducted there. Family members residing in the home include 2 adults (licensee, spouse) and 2 children. Medical Services (IMS) were discussed. Facility is currently not providing IMS during this inspection.

Main care is provided in the family, living and formal dining rooms. Children use the bathroom located next to the formal dining (hallway next to kitchen). Children have access to the kitchen/dining and backyard. Off limit areas include the home's entire upstairs (5 bedrooms, 2 bathrooms), laundry (safety knob, leads to garage) and the garage.

The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds (kept in laundry area, garage), medicine (upstairs), and hazardous items that can pose a danger to children.

Children play in the backyard. There is a concrete area for active play. There are Little Tikes and Step 2 play (with padding) equipment available for the children. The backyard is completely fenced. There are 2 cats (stay in garage). There is a barbecue grill that is covered. The right-side area has a gate (lock). The left side has a gate with lock housing the two A/C units and lawn equipment.

Per Licensee, there are no weapons or firearms of any kind in the facility at this time. The LPA did not observe any weapons. There are age appropriate toys and napping equipment on the premises. Children nap on cots, playpen and cribs in the main care area. The required fire extinguisher (2A10BC) and smoke detectors are in operable condition. The home has a Carbon Monoxide detector. No fireplace. Home has central AC and heat. The training must be taken through American Red Cross, American Heart Association, or through Emergency Medical Services (EMS) Authority approved trainers. Online courses are not acceptable and would result in a citation. The First Aid kit was observed and is complete. The First Aid kit was observed and is complete.



The licensee has current CPR and first aid that expires 11,5, 2021. The licensee has completed the online mandated reporter training at www.mandatedreporterca.com, and will renew 2/26/2021. The licensee has the required immunizations

All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years

The following was discussed with the Licensee:
Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B

The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also call the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B).

The licensee was informed that the presence of adults in the home without Criminal Record Clearance or Exemption will be cited and civil penalty assessed for $100 per day. The licensee may find additional information and forms on the DSS website at www.ccld.ca.gov including information on the Live Scan application (LIC 9163). Appointments can be made for Live Scan at 1-800-315-4507

Per the licensee, fire and disaster drills are conducted every 6 months; last drill documented and conducted on 02/03/2020.

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Emergency Disaster Plan (LIC610a), Notification of Parents' Rights Poster (PUB 394).

Licensee was reminded that supervision is always required to children in care.

Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.

Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.

--Licensee was advised to visit the CCL website (www.ccld.ca.gov) to obtain updates of courses and updates/changes to the regulations.

A copy of Safe Sleep Proposed Regulations was provided to Licensee.

LPA provided consultation during inspection.

There are no deficiencies being cited at this time, facility is in compliance with Title 22 Regulations.

An exit Interview was conducted, a copy of this Report and a Notice of Site visit was provided to the licensee.
