Licensing Program Analyst (LPA) Christopher Garlington conducted an unannounced Case Management visit to facility for the purpose of concluding the investigation into the above allegation.

LPA met with Charisse Sims, Licensee who had 10 children in care along with 2 Teachers and 1 Aide. LPA conducted interviews with Staff 1, Staff 2, Child 1, Child 2, and Child 3.

The Unusual Incident that occurred was filed as a Complaint Report on 9/16/19. The Initial 10-day visit was completed on 9/25/19 by case carrying LPA Garlington and the investigation was completed on today's date 10/09/2019.

Based upon the results of the aforementioned Complaint there is no further need to investigate the Unusual Incident Report.

A copy of this report was explained and issued to Licensee along with a copy of the Notice of Site Visit.
