On 11/30/23 at 10:00AM Licensing Program Analyst (LPA) Andrew Alemoh conducted an unannounced Annual Required Inspection at the facility. LPA met with (School Director Ariella Guillen) and toured the facility indoors and outdoors. Days and hours of operation are Monday-Friday 6:30AM to 6:00pm. Optional morning care is providing from 6:30AM to 8AM. Afternoon session begins from 1:50pm to 6:00PM. Per Director the children are picked up from elementary school and are brought back to day care center. The day care center is using 5 classrooms.

Classroom #1 is for kindergartens as well as a pickup/drop off for all day-care children.
Classroom #2 is for 1st and 2nd graders.
Classroom #3 is for 5th and 6th graders.
Classroom #4 is 3rd and 4th graders.
Classroom #5 is for 1st graders.

All classrooms contain: trash cans with tight fitted trash bags. Storage cabinets were observed to contain extra classrooms supplies and play equipment. Safe and age appropriate toys and play equipment was observed. Cubbies for children to place their belongings. A drinking igloo was observed in the classrooms with disposable cups. Per director children bring their own water bottles to the facility as well as their are no naps taken at the facility however the facility does provide mats. All classrooms contained a 3A40BC fire extinguisher to be in operable condition.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, and other hazardous items are made inaccessible to children (stored in bathroom with a lock). LPA observed the children medications were stored correctly. (Locked in a cabinet in director office).

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

There are two separate bathrooms. 2 toilets and 2 sinks. LPA observed the bathrooms to be clean. Toilet and sink and clean and in operable condition. There is no shower/tub in the bathrooms. Soap and paper towels were observed in the bathroom area.

No kitchen on site, (LPA noticed a microwave and toaster oven in the director office) kids bring their own food and snack from home. Hot lunch is an option during summer camp if necessary. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Tested and observed to be in compliance.

The outside area observed to be clean and free of debris. There is a silver iron fence as well as a black iron fence that barricades the playground area. There is a shaded area for rest and play. Toys are brought from inside the classrooms to the outside area if need be.

Last fire drill was conducted on September 29th 2023 . Mandated Reporter training expires on 11/09/24. CPR/Pediatric First Aid expires 04/27/25. LPA reminded director to update mandated reporter and first aid training every 2 years. First aid kit was observed in front of the classroom with supplies readily available.

Facility representative was reminded, Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance, transfer or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.


Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 14 children in care. LPA reviewed a sample of children’s files and observed files were complete. (LPA) reviewed a sample of staff files and observed files were complete.

Per Director Arielle children with Incidental Medical Services are taken to the elementary school nurse, and or Director Arielle. (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Exit interview conducted, the report was reviewed. A copy of this report, and the center Appeal Rights was provided to director Arielle.
