On February 27, 2020 Licensing Program Analyst (LPA) Reiko Jones-Modeste conducted an unannounced Annual Random REQUIRED inspection at the facility listed above. LPA met with Licensee Carolyn Sanford for a guided tour of the facility. Upon arrival at 10:10am LPA observed the Licensee with one infant, two toddlers and three preschooler children present in the toddler classroom. Licensee understands she is operating out of the scope of the License with an infant in care. A Licensee Assistant was observed on the preschool playground leaning equipment. LPA observed a facility roster. Licensee stated hours of operation are Monday through Friday 6:30am-6pm.

LPA toured the facility according to the sketch on file. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were observed. The following required postings were observed in the preschool classroom: Emergency Disaster Plan, Menus, Disaster Drill log and Parents Rights.

In the middle toddler classroom children were observed playing and eating snacks while Licensee performed rounds of toileting. Toys and equipment were in good repair. Toddler restroom located opposite the toddler classroom observed safe and sanitary. Front toddler classroom observed with appropriate toys and equipment. Availability of indoor drinking water was not observed
Next door to toddler restroom LPA observed the kitchen with detergents and knives inaccessible.

LPA observed the front toddler classroom not in use with with appropriate toys and equipment as well as the required postings including the Disaster Drill log.

In the rear preschool classroom upon arrival LPA noticed a remarkable odor of urine.

Individual cubbies were observed labeled with children's names. Licensee states the classroom is used for preschoolers ages 2-5 years old. Toys, games and equipment were observed along with sleeping equipment including mats with cots and blankets as well as sign in sheets. First Aid supplies were observed in the toddler and preschool classrooms.

An off-limits infant classroom with restroom attached and an adjacent infant napping room were observed as Licensee is interested in obtaining an infant License.

LPA observed the isolation restroom located in the off-limits infant classroom.



Age appropriate sinks and toilets were inspected for availability and good repair in the toddler and preschool restrooms.

LPA found fire extinguisher in compliance with service tag dated 2018; Licensee was advised to have extinguishers serviced annually. Smoke and carbon monoxide detectors found in compliance. Director provided proof of First Aid training for Licensee Assistant S2.

Kitchen, refrigerator and pantry were observed in compliance. Per the Director, lunch and snacks are provided. Food and snacks menus were not observed posted.

Children and staff records were observed incomplete with the Licensee's personnel file unavailable.

The outdoor toys and equipment were inspected for safety, good repair and age appropriateness. Required shade and fencing were found in compliance. LPA observed the toddler yard separate from the preschool play areas.

Teacher child ratios were observed in compliance. All children were adequately supervised.
LPA advised Director to review www.ccld.ca.gov for department updates and changes.

Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any licensing report documenting a type “A” deficiency. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).

The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent.

See attached deficiencies page (LIC 809D) for citations in accordance with Title 22 California Code of Regulations and/or the Health and Safety Code. Deficiencies cited must be cleared to protect the children’s health & safety.
