On 8/1/2019 at 10:10 a.m., Licensing Program Analyst (LPA), Brianna Reynoso met with above facility's Director, Angie Bustamante. LPA was at the facility for the purpose of generating the report for the unannounced annual random inspection that was conducted on 7/30/2019. During the annual random on 7/30/2019, LPA was unable to provide the Director with a copy of the Facility Evaluation Report due to technical difficulties with the program, however LPA was able to provide the Director with a print out of the Facility Visit Summary Report during the inspection.

The following is the report related to the unannounced annual random inspection conducted on 7/30/2019.

On 7/30/2019 at 12:20 p.m. Licensing Program Analyst (LPA), Brianna Reynoso was greeted by above facility’s Director, Angie Bustamante. LPA was at the facility to conduct an unannounced random annual inspection. LPA disclosed the purpose of the visit and was granted entry into the facility by Director, who also guided LPA on a tour of the facility.



The facility's hours of operation are Monday through Friday 6:30 a.m. to 6:30 p.m. Per the Director, the facility transports school age children to and from school. Per the Director, the facility provides snacks to the school age children in care, and they are not enrolled in any food program. The facility uses the School Age classroom, which can be accessed through the left entrance of the facility hallway.

During today's inspection, LPA observed 16 school age children under the care of two staff members.

Furniture and equipment were inspected for operable conditions and age appropriateness. Telephone service, heating, lighting and ventilation were evaluated. LPA observed individual storage for children's belongings. Children have access to water through a filtered water pitcher and disposable water cups located inside of the classroom. The facility uses the Director's office to isolate sick children.

Age appropriate sinks and toilets were inspected. Toilets flush properly; toilets and sinks are reachable by the children. LPA inspected two toilets and three sinks, all of which were in operable condition and met the Title 22 requirements. One restroom used by the school age children was located inside of the School Age classroom, and the second restroom was the outside restroom, however the school age children have access to this restroom through their classroom. LPA observed a separate staff restroom located inside of the facility.

First Aid supplies, smoke detector, carbon monoxide, and fire extinguishers were observed and in operable condition. The fire extinguisher (2A10BC) had a service tag attached indicating it was last serviced on 6/6/2019. Trash cans with tight lids were observed. The facility offers snacks to the children enrolled in the school age program. LPA observed snacks which were stored inside of the kitchen area, and were within expiration date. Menu was posted, dated, and accessible for parent’s review. LPA observed an allergy list posted inside of the classroom, and inside of the kitchen area.



The outdoor play area was inspected and observed to be free of hazards, loose, or sharp objects. Equipment was inspected for safety, cushioning material, conditions and age appropriateness. Climbing structures, slides, and play equipment were found to be securely anchored with resilient cushioning material underneath and around the perimeter. Children have access to water through water pitchers and disposable cups during outdoor activities, as well as water fountains. The playground is fenced all around, and no bodies of water observed in the outdoor play area. The playground area has a shaded rest area for the children. The playground is shared between the school age and preschool program, however LPA observed a granted waiver request for shared outdoor activity space, which was approved effective 9/14/2015.

Children and staff files were reviewed and found to be complete. The facility roster and fire drills were up to date, and all staff have been fingerprinted and associated to the facility. The facility Director has a current CPR and Pediatric First Aid certification, which expires on 2/2021. During the inspection LPA observed adequate teacher child ratio in the classroom. The parent board was reviewed and had all the required forms posted and accessible to parents.

The facility stores all medications inside of a locked box located near the front reception desk.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from date of employment to complete training as required. The training may be conducted at the following website www.mandatedreporterca.com

The following was discussed with the Director:


Smoking is prohibited on the premises of a child care center; Infant walkers, baby jumpers, exersaucers, baby rockers and any other item that falls into that category are prohibited; Earthquake safety and necessity of drills are to be conducted every 6 months and logged; Required forms for children’s files, facility files, staff files, and posting requirements; Fingerprint clearances and the transfer process.

The above facility was found to be in substantial compliance, and no deficiencies were cited during today’s inspection.
