On June 16, 2021 at 1:15PM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an unannounced Annual/Random inspection for the School-Aged component. LPA met with the facility Director, Monica Smith, who guided LPA on a tour of the facility in accordance with the facility sketch. Upon entry, LPA counted 9 School-Aged Children in care and 1 Teacher. There is one classroom. The facility also is licensed for Infant program with a toddler component(#197492998) and a preschool license (#197419770). The facility operates Monday through Friday from 6:00 AM to 6:00 PM. Incidental Medical Services (IMS) was discussed.

Furniture and equipment were inspected for age appropriateness and good repair. Room is clean and safe. Telephone service was verified. Heating, lighting, and ventilation are adequate. There are cubbies with coat hooks for children's belongings. LPA observed age appropriate toys and materials. Drinking water is available inside the classroom in the form of individual water bottles kept chilled in the refrigerator and a pitcher with disposable cups. LPA observed staff providing supervision to children while children watched a movie in a conditioned room during nap time. School aged children have the option to nap, but are not required to during "down time." Trash can has lid.

LPA inspected and observed two restrooms each with a sink. One restroom has a urinal and the other restroom has a toilet. Boys also use toilet in gender neutral bathroom. Toilet and sink are functioning properly and are age appropriate. LPA observed soap, toilet paper, seat covers and paper towels readily available. Water temperature is appropriate.

Children are inspected for illnesses (wellness policy) as they arrive. As a COVID-19 safety precaution, all children are temperature checked and parents complete a symptom

questionnaire prior to child entrance into facility. A review of medication policy indicated that prescription medication is administered only with parent's written permission. Medication is brought and locked at the center. Medication is properly labeled and stored in its original container. Medication is kept at the center until expiration date.

Outdoor play equipment was inspected for health, safety, cushioning material, good repair and age appropriateness. The play area has playground cushion and concrete. The area was observed to be free of debris. There is a shaded area with park benches for rest. Shaded area uses cement in buckets to anchor shade structure to ground. The buckets are cracked, but immovable. Director agreed to fasten a cushioned material to the outside to the buckets prior to allowing children outside to play. Due to inclimate weather caused by the heat wave, children have not been permitted to play outside or have access to outside play structure. Director completed declaration. Water pitcher and disposable cups are brought outside during play. Play area was inspected and found to be free of hazards and inaccessibility to bodies of water. Separate playground schedule for school-age and preschool children. Outdoor play equipment and bikes are present and in good repair.

There is a clean fully equipped kitchen (off limits) with refrigerator, freezer, stove and microwave oven. The facility provides breakfast, snack, lunch, afternoon snack, and dinner. Menu is posted in the kitchen, front office, and copies are provided to parents upon request. Allergy list is posted in the kitchen, classroom, and office. LPA observed an appropriate amount of food and snacks. The chemicals are kept separate from the food (locked with key in cabinet in classroom inaccessible to children).

Sign in and out sheets were reviewed. The parent board was reviewed and has all of the required forms posted. Fire/earthquake drills current. Roster current.

Teacher child ratios were observed and staff name recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Personal rights of children were discussed.

Children's records and staff records were reviewed. During walk through, LPA verfied name of Staff 1 and determined Staff 1 was not on association list. During record review, LPA determined Staff 1 has worked at the facility for over a year. Staff are certified in Pediatric CPR and First Aid expire 02/28/2022. Additional forms and a copy of Title 22 Regulations may be obtained at the department's website www.ccld.ca.gov.

--Director advised before an individual can obtain a childcare license or work at a licensed childcare facility, they must receive a criminal record clearance or exemption from the Department’s Care Provider Management Bureau (CPMB).

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Center Director advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation (call within 24 hours and submit written report within 7 days) and on the form LIC624. Center reminded to review updates/regulations as well as quarterly updates on Department website. The "Notification of Parent's Rights" poster must be posted in an area of the home accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.

California Code of Regulations, Title 22, Division 12 Chapter 1, 101170(e)(2) Criminal Record Clearance Type A violation is being cited on attached LIC809D.

A Civil Penalty of $500 has been assessed during this inspection for staff criminal record clearance. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.

Director was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. A copy of this licensing report (LIC809) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent. A copy of the Acknowledgment of receipt of licensing report (LIC9224) was
provided and must be kept in each child's file. In addition, any child enrolled within the following 12 months must also receive a copy of the Type A Citation.

Consultation provided during the inspection. Exit Interview conducted and a copy of Report, Appeal Rights, and Notice of Site Visit were provided to Director, Monica Smith.
