On 09/19/2023 at 11:15 AM, Licensing Program Analysts (LPAs)Katrina Chicote and Angelica Wallin conducted an Unannounced Required – 1 Year inspection to the above facility. Upon arrival, LPAs disclosed the purpose of the inspection and initially met with S1, Licensee's Assistant, who provided entry to facility. Licensee, Cupid Hall, arrived shortly after and guided the LPAs on a tour of the facility both indoors and outdoors, including off limits areas. Adults in the home were discussed and all have criminal record clearance. There was one preschooler present during today’s inspection. Licensee states that there are currently 17 children enrolled. The children's roster was reviewed and is current. Per Licensee, the facility’s hours of operation are 6 days a week 24 hours. LPAs discussed Overnight Care procedures with Licensee and Licensee understands that she cannot care for a child longer than 23 hours at a time. Required postings were discussed at time of inspection. Disaster drill log was also available during today’s inspection, last disaster drill conducted on 09/14/2023.

This is a single-story home which consists of three bedrooms and two bathrooms, Kitchen, Dining Room, Living Room, and attached garage. There is a fireplace in the home that is barricaded. Main Care Area used by the children include the Living Room (located upon entry of the home) and Bathroom 1 (located down hallway in between bedrooms). Per Licensee, areas off limits to children and parents include all bedrooms, Kitchen, Dining Room, and attached garage. LPAs observed there is a gate available to block access to Dining Room/Kitchen. LPAs observed a door through Dining Room that leads to attached garage that has operable child safety knob making this area off limits. Per Licensee, off limit areas are also locked during operating hours, LPA observed doors to be locked at time of inspection.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. LPAs observed ceiling fans through out the home. There is a working telephone maintained in the home. At 11:20 AM, LPA observed

Living Room to have couches and other decorative items. LPAs observed a preschooler sleeping in a crib in the back of couch where storage for toys are located. At 11:22 AM, LPAs observed three bins with various toys available for children and additional storage area across from this area that is behind a bar counter. At 11:25 AM, LPAs observed this storage area to be cluttered and have various items such as toys, art materials, puzzles, and other items stacked up high and leaning haphazardly. LPAs discussed this observation with Licensee, and Licensee states this area is off limits to children though LPAs did not see any barrier making this area off limits to children at time of inspection. At 11:34 AM, LPAs observed Bathroom 1 to have operable toilet and sink and to have general area safety and sanitation at time of inspection. LPAs observed medicine cabinets and lower cabinets in Bathroom 1 and did not observe any hazards. LPAs observed storage area in rear of Bathroom 1 to contain additional bathroom items. LPAs observed cabinets in rear storage area to have toiletry supplies stored up high. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. LPAs observed these items to be stored in walkway to off limits garage where washer and dryer is located. The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

Per Licensee, the children will have access to front yard area for outdoor play. LPAs observed a porch area accessible through sliding glass door in Living Room to have plastic crawl tube, lego table, and other children's toys available. Per Licensee, this area is currently off limits to children in care at this time until construction is made to repair area. LPA notified Licensee that this area cannot be used until said repairs are made and that it needs to be reported to Department prior to beginning construction on area. LPA observed a large breed dog kept in porch area, per Licensee dog is kept separate from children at all times. LPA observed front yard has grass area and concrete through out. LPAs observed adequate perimeter fencing through-out the yard though there is a driveway gate that can be opened. Per Licensee, she is outside supervising the children when children are playing outside. LPA advised when outdoor area not adequately fenced, as is the case when driveway gate is opened, Provider must be with children at all times when outdoors. At 12:00 PM, LPAs observed dome climbing equipment pushed to side with no cushioning material below to absorb fall. LPAs observed cushioning material below dome to be puzzle mat cushioning that was broken up not providing cushioning at time of inspection, per Licensee children take this apart daily. LPAs observed plastic play houses to have visible dirt and spider webs and plastic play equipment to have broken pieces exposing sharp edges at time of inspection. At 12:04 PM, LPAs observed dog feces on grassy area, Licensee states that dog feces is typically cleaned up daily. LPAs observed garage door to not have lock at time of inspection making it accessible, per Licensee there is a lock that will be placed back there.

Per Licensee, they provide food for children in care and use Living Room or outdoor play area for eating, if weather permits. LPAs observed child size tables and chairs available in Living Room and in outdoor play area.

At 11:40 AM, the valve on the required 2A 10BC fire extinguisher indicates fully charged but with service tag date for 02/16/2022. LPA tested smoke and carbon monoxide detector which were observed to be in operable condition at time of inspection. LPA observed First Aid kit stored in rear Bathroom 1 storage area and was inventoried for necessary supplies. The Licensee has current Pediatric First Aid and CPR valid until 02/2024. Proof of immunization against influenza, pertussis, and measles was readily available during today’s inspection for Licensee. Licesee's Assistant did not have proof of immunization against MMR at time of inspection. The Licensee and Assistant has taken the required AB1207 Mandated Reporter Training completed on 03/12/2022.
— Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPAs observation did not see any bodies of water around the premises or firearms/weapons at time of inspection.

The following were discussed:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee (or facility representative) was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility.

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed and issued the LIC 311D - Forms/Records to Keep in Your Family Child Care Home. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Licensee was advised how to access forms and Regulations online at www.ccld.ca.gov. Licensee was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.



AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com/

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

MyChildCarePlan.org – Centers and Family Child Care Homes Licensee [or facility representative] was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Megan’s Law - Family Child Care Homes

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



Exit interview was conducted and report was reviewed with the Licensee, Cupid Hall.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
