Licensing Program Analyst (LPA) Denise Miranda conducted an unannounced annual/random inspection to the facility. Upon arrival LPA met with Kristyn Garcia, Director Assistant. Licensee/Director was not present.

The facility is located on a Hancock Elementary School (LAUSD) school site and uses the auditorium and cafeteria as the indoor space and all areas of the playground for outdoor activities.
Per Director assistant the room #32 and #33 were no longer use by the program. Per Director Assistant room#26 will be use after the program get approval from the fire inspection and regional office.

There is an outdoor space immediately outside the auditorium with benches and tables that is being used for outdoor activities and/or eating snacks. There are no bodies of water on the premises and the outdoor space is completely fenced in. The licensee states that the children in the program will not be allowed to use the yellow and green play equipment (monkey bars), as well as the monkey bars on the purple equipment. The after-school program from: Monday- Wed, Thursday and Friday from 2:15pm to 6:00pm and Tuesday 1:15pm to 6:00pm.
LPA observed staff#1 First Aid and CPR training to expire on 05/2020.
LPA also observed 124 children in care with 13 staff and director assistant.

Furniture in each classroom was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. Drinking water is readily available in the classroom.
Facility is operating within capacity limitations and compliant with ratios.
The following were observed/discussed during this inspection:
1. There are no bodies of water observed on the facility;
2. Fingerprint clearance of staff;
3. Teacher to child ratios;
3. Care and supervision of children in care; teacher/child interaction;

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.


The facility was found to be not in compliance per Title 22 regulations, Type B deficiency will not be cited today 9/24/19. A copy of this report along with a Notice of Site Visit were issued and explained to the director assistant. An exit interview was conducted.

Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. The ADA requires that child care providers not discriminate against persons with disabilities on the basis of disability, that is, that they provide children and parents with disabilities with an equal opportunity to participate in the child care center's programs and services. Requirements for fingerprint clearances and associations were discussed with the licensee.
Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12 month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed. Medication is properly labeled and stored in the original container. All medication is stored in parents resource room office. First Aid supplies are stored in each classroom.
Children and staff records were reviewed for completeness records were found to be complete.
Lead Teacher advised that records for all children and staff must be maintained for 3 years after separation from the facility. A record of staff names was check against current facility roster, Staff are currently associated to the campus in which they are assigned to work.

The following was discussed with the licensee:
The Director assistant was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. 4. Availability of drinking water to children both indoors and outdoor; there are water fountains in the playground areas.

The following forms were observed to be posted. The Facility License, Parent's Rights Poster (PUD 393), and Emergency Disaster Plan (LIC 610).



Inspection of the outdoor play area(s) was conducted. Program use the elementary school playground. There is adequate shade for all the children in care. Fencing around the perimeter of the play area is at least 4 feet high. Playground is free from miscellaneous debris such as tree branches, trash, leaves, etc.

The following was discussed with the licensee:
Mandatory Forms for the children’s files and provider’s files, Requirements for fire drills, earthquake drills and documentation for both.
The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.
Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.
