13 | On April 12, 2022 at 8:39AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an unannounced complaint inspection to deliver findings on the above complaint allegations. LPA disclosed the purpose of inspection and was granted entry by Director Elessia Session, who guided LPA on a tour of the facility. Upon entry, LPA observed 0 children in care, as school age children are still attending school.
During the course of the investigation, LPA interviewed staff, children, parents, and obtained facility records. Based on evidence collected, the facility van obtained a flat tire during the normal scheduled after school pick up times. The Licensee arrived to the scene to pick up the children who were in the van and take them to the facility. The Licensee returned to Director to assist her with the flat tire, which took approximately 15 to 20 minutes to repair to enable Director to drive back to the center. Licensee was required to purchase 4 new tires with receipt of purchase dated 2/11/2022. |
