On 01/10/2020 at 12:40pm Licensing Program Analysts (LPAs) Ericka Hill and Lourdes Castellanos conducted an Unannounced Case Management Investigation Visit to follow-up on the information received from previous Complaint Report received on 10/29/2019. LPA's were greeted by the Director, Aasritha Puram (S1) and guided to S1's office. LPA's informed S1 the purpose of the visit was to follow-up on the information received regarding school-aged children attending the facility during the summer session. LPA's observed the facility ratio and observed 21 children supervised by 4 teachers (3 teachers and 1 Director).

LPA's were informed that the school-aged children attend the facility only during the summer hours for approximately 4-5 days and 1-2 hours throughout the entire summer. S1 stated that the facility has only allowed school-aged children to attend the facility during the summer of 2019. S1 and S2 stated that the school-age children are between the ages of 8 and 13 years old and are the children of the teachers instructing at the facility. Director stated that the activities the school-age children engage in include reading, play on the computer, going outside in the backyard playground, sit in the kitchen area, and sit in the office. Director stated that the school-aged children do not engage with the preschool children often but will help teachers with clean-up after activities. Director also stated that the school-aged children never instruct the preschool children or are never left alone with the preschool children. Director stated that the facility hosts Holiday events that allow children that are not enrolled in the facility to attend the Holiday events.

In regards to the photos, Director stated that the photos are taken by the Director or a teacher at the facility. Director also stated that the photos are shared through "Group Chat" between the teachers and the photos are posted on the facility facebook page. During the investigation the Director verified that the 3 photos provided were in fact on the facility facebook page. Photos #1 and #2 was taken on July 6th during a Reptile show titled the Reptacular Animals. Director stated that the Reptile instructor is certified and the facility hosts this event every summer. Photo #3, depicting the older school-aged child, was taken on July 3rd during an art activity were S2 was instructing the preschool children on how to paint an Eiffel Tower.

Based on the interviews conducted and a preponderance of evidence obtain it was determined that S1 was allowing school-aged children to attend the facility during the summer session of 2019. This is a violation of Title 22 Regulations.

A deficiency as cited and Type B citations will be issued today, 01/10/2020. See 809-D for deficiency cited.

An exit interview was conducted and a copy of this LIC809, LIC809-D, Appeal Rights (LIC9058), and a Notice of Site Visit (shall remain posted for 30 days) was provided to Aasritha Puram, Director.
