On 11/06/19 at 9:30am, Licensing Program Analysts (LPA's) Lourdes Castellanos and Ericka Hill arrived at the facility to conduct an annual random inspection at the facility. LPA's met with Aasritha Puram , Director who guided LPA's on a tour of the facility. LPA's Castellanos and Hill toured and inspected the facility as identified in the facility sketch. The hours of operation are from 7:00am to 6:00pm, Monday through Friday. The facility is operating within capacity limitations. LPA observed a total of 16 children being supervised by 1 teacher and the Director. LPA's observed 1 additional staff arrive to the facility at 10:00am. ALL STAFF ARE ASSOCIATED TO ADMINISTRATIVE FILE.

LPA's observed the following during the inspection:
The facility has one large classroom with Directors office between the children's indoor activity area and storage closet (containing children's cots and video camera monitor). LPA's observed age appropriate furniture, equipment, toys and materials for the children. The classroom was observed to be clean, safe, and free of any Health or safety hazards. Telephone service was verified as well as adequate heating, lighting, and ventilation. Children's belongings are kept in cubbies along the wall as you enter classrooms. Drinking water is available inside the classroom in the form of water dispensers to refill children's personal water bottles. LPA's also observed 5 cameras inside the building and 1 in the outdoor activity area.

The children's bathrooms are located in the back of the facility near the outdoor exit door. One bathroom has a toilet and a sink, while the other has only one toilet with the sink outside the bathroom door. The staff restroom is located in the kitchen area with a gate barrier blocking children's ability to enter the kitchen area. LPA’s observed the bathrooms to be clean and sanitary, with soap, toilet paper and paper towels readily available. Toilets and sinks are functioning properly and age appropriate.

Outdoor play equipment was inspected for health, safety, good repair, and age appropriateness. Center utilizes sand area and climbing structure is cushioned with rubber matting and there is sufficient and adequate shade available. Large play equipment and climbing structures are securely anchored. The area was observed to be free of debris and free of hazards, loose and /or sharp items. LPA's did observe rocks that children use during play activities in which LPA's informed the Director to always make the rocks inaccessible when children are not using them. LPA's observed the Director placing the rocks above an outside shed, inaccessible for children to reach. There are no bodies of water observed on the premises. The playground is enclosed by a fence and brick walls keeping the children safe while playing in the outdoor activity area.

Food preparation area/Kitchen. Kitchen is equipped with a full-size refrigerator and sink with hot and cold running water. LPA's inspected the kitchen for safety, cleanliness, proper equipment & protection against contamination and storage. Center does provide snacks daily, menus observed and posted. Disinfectants, cleaning solutions, poisons and other items that could pose a danger to children were stored/locked and inaccessible to children.

The Parent Board (located in the main entrance area). LPA's observed all documents that are required to be posted according to Title 22 Regulations, except the Fire Drill Log. LPA's informed Director to post the Fire Drill log on the Parent's Board.

Records. A sampling of Children's and Staff records were reviewed as part of this inspection. Two staff are certified in Pediatric CPR and First Aid, expiring 2021. The children files reviewed today were complete including current/required immunization's. Sign in and out sheets were inspected and contain authorized representative signatures.

Detectors and Kits. LPA's observed a fully stocked first aid kit, fully charged fire extinguisher, and a carbon monoxide detector in the center. Upon inspection, LPA's advised the Director to purchase and set-up a smoke detector before the LPA's exited the center, since LPA's did not observe a smoke detector in the facility. LPA's witnessed the Director setting up and testing the smoke detector. According to the licensee/director, the facility does not provide Incidental Medical Services (IMS).

The following was thoroughly discussed with the licensee:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag.

Requirements for disaster drills and mandatory documentation for all children and staff files were discussed. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Child Care Center online at www.ccld.ca.gov. Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care Center online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.



Licensee was made aware of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Assembly Bill 1207: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at: www.mandatedreporterca.com.

There were no violations noted as a result of this inspection, Center is operating in accordance to Title 22 Regulations. Copy of LIC 811(Confidential Names List ) was provide during this inspection.

An exit interview was conducted with the Director. A copy of this report along with a Notice of Site Visit were issued and explained to the licensee /Director Aasritha Puram. The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days.
