Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Required - 1 Year Inspection. LPA met with Director Gwen Bias who assisted analyst with a tour of the facility. This is an Infant Program serving infant children (Birth to 2 years of age). The Infant center (indoors and outdoors) is physically separate from a preschool program housed on the premises. There is no commingling with other programs. Facility operation hours are Monday through Friday from 2:00 AM to 6:00 PM.

All areas identified on the Facility Sketch were inspected. At approximately 12:50pm LPA observed the following ratios during the inspection: Infant Room: Three staff caring for nine infants. LPA observed all furniture and equipment was inspected for age appropriateness and good repair, telephone service, heating, lighting and ventilation were evaluated. Storage for infants’ belongings and napping equipment/cribs were inspected and found to meet all regulatory requirements at this time. The changing table has raised sides at least three inches high and is within an arm’s reach of a hand washing sink. Toys were observed to be clean. There is plenty of drinking water in the classroom available as needed and children have their names labeled on their bottles.

LPA observed a first aid kit kept kept in the classroom. Carbon monoxide detectors and smoke detectors are present in the facility. 2A10BC Fire extinguishers were serviced in February 2022. Hazardous items including including medicine and cleaning compounds are stored inaccessible areas such as high cabinets and inaccessible office cabinets. to children.



At this time, the office is used as an isolation area. There are portable cots and changing mats for infants/ to rest and be changed if needed. The facility provides lunch and AM and PM snacks. Some parents provide breast milk for infants. The parents bring sheets and blankets and they are sent home at the end of each week to be laundered. Food preparation areas were toured for safety, cleanliness and proper equipment.

Outdoor equipment was inspected for safety, cushioning material, good repair, and age appropriateness. LPA observed the outdoor area to have required shade, available drinking water and adequate fencing. LPA observed the play area appear to be free of hazards or bodies of water. LPA Birks reminded Director Bias that Infants need to be within the direct care and supervision, including visual supervision of staff at all times.

LPA observed the classroom to be within the required Teacher infant ratio. LPA evaluated care and supervision to determine if the basic needs of infants were met and appropriate. LPA observed that the facility is missing needs and services plans for Infants. LPA conducted a Children's Records Review for completeness and to ensure children have proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights etc. on file. LPA observed Child #9 missing immunization records. Children's roster was reviewed and is current.

LPA informed Director Infant Needs and Service Assessments are to be done quarterly and signed in by Parents and/or guardians. At approximately 3:00pm Staff Records were reviewed to ensure the following: All individuals present have obtained a criminal record clearance or criminal record exemption, documentation of education credits, Criminal Record Clearance documents available, Verification of current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination available. LPA observed at least one staff member to have current CPR (Expiration 2023) .

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following was discussed with the Director Bias : Recently Approved Safe Sleep Regulations in Effect (PIN 20-24-CCP). LPA also provided advised to sleep infants on their backs and check on them at least every 15 minutes. LPA provided a copy of PIN 20-24-CCP to Director Bias.

Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited. No infant walkers, no Johnny Jumpers, no excer-saucers or any other item that falls into that category are allowed in facility. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.


Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. Records for all children and staff must be maintained for three (3) years after separation from the facility. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Note: LPA observed multiple infant swings in the facility. LPA informed Director Bias that children must be removed if they are falling asleep in the swing.

Exit interview was conducted with Director Gwen Bias. The Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
