Licensing Program Analyst (LPA) conducted an unannounced Case Management Inspection to the above facility. The purpose of this inspection is due to the facility permanently closing effective today 5/19/23. Licensee, Ashleigh Pratt informed Child Care Licensing (CCL) on 5/9/23 of intent to close the facility including the Preschool License on the premises (Lic #197420071). A signed letter from Licensee, Ashleigh Pratt was received by CCL on 5/12/23, confirming the facility closure as of 5/19/23.

Upon arrival LPA met with Gwen Bias, Director, who provided a tour of the facility. There were 10 infants present with three (3) staff. Staff #1 Tatiana Tejada, Staff #2 Melva Harding and Staff #3 Christina Lyles were supervising children.

Per Gwen Bias the parents of all children enrolled have been contacted and are aware of the closing of facility as of today 5/19/23. LPA observed and collected a copy of the notification sent to the parents of enrolled children. Per Director, parents have been provided with resources in finding daycare at alternate locations.

LPA provided Technical Assistance to Director and provided information on applying for a Center License a. LPA collected facility records including facility license and requested personnel roster during visit.

LPA reminded the Acting Director that originals or photocopies of all children's records shall be kept for at least three years following termination of service to the child.
