On 04/19/2024 at 9:00 AM An Unannounced Required – 3 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and initially met with S7 who allowed entry to facility and provided guided tour of the facility both indoors and outdoors. Facility is a Preschool Program with a Toddler Component currently licensed for a capacity of 64 (52 Preschoolers and 12 Toddlers). The program currently operates Monday thru Friday from 7:30 AM to 5:30 PM. Required postings were observed posted on bulletin board outside to the right upon entry of facility.

The facility is made up of four classrooms: Green Room (3 year olds), Blue Room (Toddlers 18-24 months), Purple Room (2 year olds), and Red Room (4-5 year olds). At the initial start of the site visit, LPA observed 11 preschoolers with two teachers (S1 and S2) in Green Room, 14 preschoolers with two teachers (S3 and S4) in Purple Room, 20 preschoolers with three staff (S4, S6, and S7) in Red Room, and nine toddlers with two teachers (S3 and S5) for Blue Room which LPA observed were playing in Toddler Play Yard upon arrival.

Beginning at 9:05 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and age appropriate tables and chairs available for preschoolers and toddlers well maintained at time of inspection. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area available for quiet play. LPA observed variety of toys available for children to be age appropriate in toddler classroom. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected. Per S7, linens are provided by the facility and taken home weekly for washing. Availability of drinking water was reviewed. LPA observed children's personal water bottles available in each classroom, and per S7 the water bottles are refilled through their water filtration system. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toileting supplies, area safety and sanitation. LPA observed Bathroom 1 available through Red Room that had three toilets and two sinks and Bathroom 2 with three toilets and three sinks that

is shared by Purple Room, Blue Room, and Green Room. LPA observed Bathroom 2 is shared by Preschool and Toddler component and accessible through Purple Room and Blue Room. LPA observed two Kaola Crate wall changing tables in this bathroom for use by Toddler Program. LPA observed trash cans used to discard food have tight fitting lids at time of inspection.

LPA observed a first aid kit is kept on high shelf in each classroom and was inventoried for supplies. LPA observed smoke alarm system and fire extinguishers present at facility. LPA discussed carbon monoxide detectors present at facility which is part of fire alarm system but was not able to test if operable at time of inspection. LPA did observe smoke alarm system with light and notice that it is operable at time of inspection. Disaster drill log was available and last drill conducted on 10/23/2023. Sick/isolation area is in corner of Red Room where there is a mat. Hazardous items including poisonous cleaning compounds were stored inaccessible to children, LPA observed these items stored on high shelves at time of inspection. LPA observed medications stored in Red Room for four preschoolers (C1, C2, C3, and C4) stored according to Title 22 Regulation requirements with proper documentation in children's files at time of inspection.

Snack/lunch menus were reviewed and observed posted in each classroom. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. Facility provides Breakfast, Lunch, and PM Snack.

At 9:45 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed Preschool and Toddler Component to have separate outdoor play yards. LPA observed Toddler Yard to have surface that is maintained with a variety of age appropriate toys free of loose and sharp parts such as ride along toys and sensory table and toys. LPA observed Preschool Yard to have large age appropriate play structure with slides, steps, and climbing equipment with a turf grass cushioning material below that are all well maintianed at time of inspection. LPA observed concrete bike path with bicycles available for preschoolers that are maintained. LPA observed a sandbox with toys in Preschool Yard and discussed cleaning and sanitation protocols with S7. LPA advised FR that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA observed children have access to a drinking fountain with water bottle filler in outside play yard. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Facility uses electronic sign in and out system, and sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA at time of inspection.

Beginning at 10:30 AM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. CPR card expires on 05/26/2025 for S3. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today.
A notice of site visit was given and must remain posted for 30 days.
