On 10/4/2022, Licensing Program Analyst (LPA), Loyce Phillips met with Licensee, Latanda Guy today for the purpose of conducting a Case Management-Other inspection. During this unannounced inspection, LPA observed 3 children in care.

The purpose of todays visit is to ensure Licensee is aware and have received notice of the Exemption Denial for Dreon Edwards. Licensee acknowledge the Exemption Denial letter and stated an appeal process is in progress.

Licensee stated Dreon Edwards is her son and does not reside in the home nor is he employed at the facility. LIC 300B and Declaration on file.

An exit interview was conducted. A copy of this report, notice of site visit were discussed and provided to Licensee, Latanda Guy.
