On November 3, 2021 at 10:46AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an announced Case Management - Licensee Initiated inspection to inspect the new installed play ground structure at the facility. Upon entry, LPA met with Director Karen Allen who guided LPA on a tour of the play ground area. During inspection, 157 children were present.

At 11:00AM, LPA observed the caterpillar, lady bug, and mushroom climbers in good repair with no loose or sharp parts. Play Structure A located on the west side of the play ground has 3 slides, 1 climbing structure, 1 ladder, and 1 stair case that leads to the top level. Activity space is both on top and underneath the structure. LPA observed the structure to be in good repair with no observable sharp parts or hazards. Play Structure A the caterpillar, lady bug, and mushroom climbers are on top of brown and black playground cushioning material. A Large Sail Shade provides observable shade to the play structure and climbing structures listed above. The play structures listed above are also anchored to the ground.

At 11:15AM, LPA observed the Music Station in the center of the play ground including a chime harp, xylophone, and bongos. The Music station is not covered by shade. The chime harp and xylophone have a coating that prevents the toys from getting to a dangerous temperature. LPA provided consultation to provide shade for the music area especially during the summer months when it gets to hot temperatures. LPA felt the Music Section toys and they were not at an observably warm temperature to the touch. The bongos have a black bottom which was observably hot to the touch during inspection. LPA provided consultation to cover black part of bongos or provide shade to that area. Top of bongos where children will touch was not observably warm.

At 11:25AM, LPA observed the Rock Wall Climber, the blue and green climbers, and the grey, blue, and orange steps in the center of the play ground. The climbers and steps are located on a black and green play ground cushion which is observably softer than the brown cushion for fall impact. LPA provided consultation for 1 child to use the top of the Rock Wall Climber at a time to prevent possible risk of children pushing or falling on top of each other.

At 11:35AM, LPA observed Play Structure B on the east side of the play ground. Play Structure B has 2 slides, 3 climbing structures, and 1 staircase that leads to the top of the play structure. Activity space is available on the top and bottom of the structure. LPA observed the structure to be anchored to the ground, sturdy, and without any observable sharp parts or hazards. Play Structure B is on top of brown and black play ground cushioning material. An additional Large Sail Shade is providing shade to Play Structure B. The age appropriateness of the play structure is posted on the structure as well. Prior to inspection, Director submitted the manufacturer's specifications to LPA which stated the age appropriateness for all the structures, climbers, and outdoor toys is from ages 2 to 5.

At 11:40AM, LPA observed the the play ground rules posted at the play ground which were installed by the company who built the play ground.

Director disclosed staff have been informed 1 staff shall provide supervision to 1 structure at a time (for a total of 3 staff present for supervision). Should 1 staff be present with 12 or less children, the children will be permitted to play at 1 structure, then move to the next station to provide proper supervision to children.

Based on LPA's observations, the play structure is in good repair and ready for children to utilize. There were no observable violations of Title 22 Regulations, and no citations will be issued during the inspection.

An exit interview was conducted, this Report, Appeal Rights, and Notice of Site Visit were provided to Director.
