Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced annual required inspection. LPA met with Lucy Bryant (Collins), Site Supervisor, who guided analyst on a tour of the facility. This is a preschool program which consists of 1 large classroom. Facility operation hours are Monday through Friday from 7:30 AM to 5:30 PM.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: 6 staff with 13 preschoolers present. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly by the child's representative. Napping equipment (cots) were observed stored in classroom. Per Site Supervisor, the isolation area is located in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classroom. LPA observed that this facility does have a carbon monoxide detector.


Disinfectants, cleaning solutions and other items that are dangerous to children, were stored in a locked cabinet in classroom, inaccessible to children. Site Supervisor states that there are no poisons stored at the facility.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste have tight-fitting covers that are kept on, and in good repair. -------CONTINUES PAGE 1 of 3

Site Supervisor stated all food items are delivered to the facility each day from the Rita Walter's facility located on 85th and Vermont. Facility provides children Breakfast, Lunch and Snack.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA did not observe any high climbing equipment during this inspection. There is adequate shade in the play yard. Availability of outdoor drinking is available by water jug and cups that are taken onto the playground by staff during outdoor activity time.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed for completeness; Inspection of required forms was made. LPA issued the Children’s Record Review (LIC 857) to the Site Supervisor during this inspection. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

LPA also issued the Review of Staff records (LIC 859) to the Site Supervisor during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in with the exception of one child. LPA advised site director to remind children's representative of the importance of signing their child in and out on the sign in sheet each day. Disaster drill log was available, last drill was conducted on 07/05/19. (Fire Drill)

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request.

First Aid supplies were observed in classroom. According to the Site Supervisor, no child is receiving medication during this inspection.

Incidental Medical Services (IMS):
IMS. Per Site Supervisor, there are no children enrolled that require IMS at this time. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised the Site Supervisor to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA received Site Supervisors email during inspection to be added to The Child Care Advocate Program (CCAP) distribution list to receive quarterly updates and regulations requirements.

Site Supervisor was made aware of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Per California Code of Regulations Title 22, Division 12, no deficiency was cited during today's inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Lucy Bryant, Site Supervisor including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. 3 of 3
