On July 13, 2021 8:32AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an unannounced Annual/Random inspection. LPA disclosed the purpose of inspection and was granted entry by Director Ana Garcia. This is a combination center that also has a preschool (197492944) and school-aged component (197492945). The center’s hours of operation are 5:00AM to 12:00AM, Monday through Sunday. Infants are located in the rear of the center physically separate from the other components. Upon arrival LPA counted 4 infants and 2 staff. Incidental Medical Services (IMS) policy was discussed.

Furniture and equipment were inspected for age appropriateness and good repair high chairs and feeding tables have broad based legs, chair trays lift and lock, high chairs are washable, changing table padding thickness in regulation and is within arm's length of the sink. Potty training is done for infants starting at 18 months. LPA observed staff during snack, diaper change and free play time. Bedding is washed weekly and returned to their cubby. The bedding is stored separately and properly inside the cubbies. There is a Infant Daily Report that identifies infant feeding schedule, diaper changes, and napping schedule. Used diapers are put in trash container with lid. Each child has container labeled with their name with personal items. Infant room is clean and safe. During inspection, LPA observed 2 baby swings in the infant sleep room not in use. During file review, LPA determined the facility does not have an Infant Sleep Plan (LIC9227) completed for each infant. Director stated the teachers physically monitor the infants during nap time every 15 minutes, but do not document. During inspection, Director created a sleep plan checklist to document infant sleep patterns.

Telephone service was verified (cordless and intercom system). Heating, lighting, and ventilation are adequate. There are cubbies for children's belongings in the infant room labeled with their names. LPA observed age appropriate toys and materials. Drinking water is available inside the classroom and provided using sippy cups and bottles.

LPA observed a sufficient quantity of cots and cribs available for napping children. LPA inspected and observed changing table (within arm reach of a sink), observed soap, toilet paper and paper towels readily available.

Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Due to inclement weather caused by the current heat wave, all children are not permitted to play outside or use outdoor play equipment. There is a cushioned area for active play. The area was observed to be free of debris. The infant play yard is shaded by a tarp. Director stated they will be replacing the tarps this weekend prior to permitting the children outside to play. Drinking water is available (Staff have gallon jugs and provide water to infants in sippy cups). Staff inspect outside play area prior to children exiting to the play area. Play area was inspected for hazards and inaccessibility to bodies of water. Outside area is completely fenced. The infant play area is separated from the preschool and school-aged area by a fence/gate.

Children are inspected for illnesses as they arrive. Staff check the temperatures of children upon entry and record the temperature. A review of medication policy indicated that prescription medication is administered only with parent's written permission (licensing medication form - LIC9221 - also used). The Director administers medication and documents the dosage, date and time onto a log. Medication can be brought and taken home by the parent daily. Medication is properly labeled, stored in its original container, and locked with lock and key. There is a separate area for isolation and care of ill children at the main entrance in office area.

There is a kitchen area with refrigerator and microwave. Chemicals (inaccessible) and food are kept in separate cabinets. Parent can bring breast milk (stored in the refrigerator and labelled) --currently no parent bringing breast milk. The center provides breakfast, lunch, PM snack, dinner, and 7PM snack (baby food meals). Menus are posted in the lobby and kitchen. Allergy lists are posted in kitchen. LPA observed an appropriate amount of food and snacks for infants during the inspection.

Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Infant file is missing Needs and Services Plan.

LPA advised Director to send Needs and Services Plan form home with parents to fill out and place in infant files. Children's records and staff records were reviewed. During file review, LPA determined Staff 1 is missing units in an infant specific course, but qualified as a preschool teacher. Director rotated a teacher with infant units with Staff 1 to adhere to ratios.

Sign in and out sheets were reviewed. LPA observed Licensing and Parent boards to be incompliance including the required posting (menu, daily schedule, community activities, licensing required documents). Fire/earthquake drills current. Roster current and maintained. First Aid Kit observed. CPR/First Aid expire 02/06/2023. Note: The training must be taken through American Red Cross, American Heart Association, or through Emergency Medical Services (EMS) Authority approved trainers. Online courses are not acceptable and would result in a citation.

-Director advised before an individual can obtain a childcare license or work at a licensed childcare facility, they must receive a criminal record clearance or exemption from the Department’s Care Provider Management Bureau (CPMB).

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
