Licensing Program Analyst(s) Maddox and Tsutaoka met with Director, Ana Garcia for an unrelated inspection, during the course of that inspection, LPA's observed the following in the School age room (pictures taken):

*There are holes in the wall
*The epoxy resin/glaze on the flooring is worn out throughout the School age room
*There are exposed wires hanging from the ceiling due to water damaged ceiling tiles
*There was a mop bucket with standing water that contained a cleaning solutions located by the children's bathroom

The above observations do not contribute to a Healthy or Safe environment for children in care., therefore, center was cited a Type B citation today under Buildings and Grounds
