13 | On March 3, 2022 at 12:45PM, Licensing Program Analysts (LPAs) Brigitte Tsutaoka and Donna Maddoz conducted an unannounced complaint inspection of the above allegation. LPAs disclosed the purpose of inspection and were granted entry by Director, Ana Garcia, who guided LPAs on a tour of the facility. Upon entry, LPA observed 1 school-age child in care. At 3:50PM, 23 school age children arrived for a total of 24 school age children in care.
At 1:50PM, LPAs were escorted outside by Assistant Director (AD) to inspect facility van. LPAs observed all seats had appropriate seat belts, no flat tires, current registration (Expires 10/2022), and current insurance (Expires 3/17/2022). LPAs requested AD press on the break to turn on the break lights. LPAs observed the break light on the driver's side is out and did not illuminate when AD pressed on break. |
