On 01/21/2022 at 8:30 a.m. Licensing Program Analyst (LPA) Deborah Lowe, conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Staff #1 upon arrival and toured the facility indoors and outdoors. Days and hours of operation are Monday – Friday 8:00 a.m to 5:00 p.m.

At 9:04 a.m. Owner/ Director, Nilofer Quaid arrived at facility.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is in need of repair to the asphalt. All toilets and handwashing facilities are in safe and sanitary operating condition. Laminate floors in the facility are clean and safe, carpeted areas are in need of cleaning. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, and other hazardous items are made inaccessible.

All kitchen, food preparation and storage areas are in need of cleaning, kitchen was observed to be free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Kitchen was observed to be in need of cleaning and removal of paint and cleaning solutions. Poisons were observed in a locked storage cabinet under Kitchen sink. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. LPA did not observe a working telephone service at the facility.

There is no swimming pool or other bodies of water on the premises. No high climbing equipment, swings or slides were observed. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements, LPA was able to hear a successful test.

LPA advised prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day, LPA observed sign in/out sheets for facility and subsidized program. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child. LPA observed 1 child’s file to be in need of medical assessment and 1 child’s file unavailable for review. LPA reviewed a sample of staff files and observed files were incomplete, 1 file in need of health screening, 1 file in need of immunization records for influenza, pertussis and measles. LPA reviewed staff files and observed files showed documentation of completed mandated reporter training. Menus are posted at least one week in advance where an authorized representative can view them.

Incidental Medical Services (IMS) are not currently being provided. Per Licensee facility does not administer medications. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are being cited: (see next page, 809 D)

LPA Deborah Lowe informed licensee Nilofer Quaid that this report dated 01/21/2022 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Deborah Lowe informed the licensee Nilofer Quaid to provide a copy of this licensing report dated 01/21/2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Licensee was provided a copy of their appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

During visit Owner/ Director Nilofer Quaid left facility from 9:27 a.m. and returned at 10:10 a.m.

Exit interview conducted and report was reviewed with the licensee Nilofer Quaid.
