Licensing Program Analysts (LPA) Lady King-Lewis met with Maria Cuevas, Center Director today for the purpose of conducting an unannounced Annual/Random inspection for the preschool license facility. Director toured the facility with LPA. There are 31 children present during this inspection with 3 teacher and 2 classrooms. The hours of operation are 6:30 am - 6:30 PM Monday -Friday. The Center also has a infant license for ages 0-2 years. There are no bodies of water. Firearms and other weapons are not allowed or stored on the premises of a child care center. During LPA inspection all children were under supervision, including visual supervision, of a teacher at all times.

Disinfectants, cleaning solutions, poisons and other items that are dangerous to children, are inaccessible to children. Storage areas for poisons are under sink with kiddy lock and in staff bathroom.

Center Director states Center does not Store or administer medication to children. Director states parents has to administer all medication.

All toilets, and sinks are in safe and sanitary operating condition. All floors are clean and safe. All kitchen, food preparations and storage areas are kept clean, free of litter, rubbish, and free of rodents and other vermin. All food is protected from contamination, and director states contaminated food is discarded immediately. All storage containers for solid waste, including moveable bins, have tight-fitting covers that are kept on, and in good repair.

Menus are posted. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Uncontaminated drinking water is readily available both indoors and out. Outdoor activity space surfaces are free of hazards, play equipment is in good condition and cushioning material to absorb falls around play equipment is in place and in good repair.

There is a separate, complete, and current record maintained for each child enrolled in the child care center.

Prior to working or volunteering in a licensed child care facility, all individuals subject to criminal record review have obtained a clearance or criminal record exemption, or clearance and have been associated to the facility. Staff records contain appropriated documentation of education credits. Staff shall complete the Mandate Reporter training with in the next 30 days. Staff 1 and 2 shall provide proof of immunization against pertussis(TDAP), Measles(MMR) and influenza. LPA verified at least 1 present staff is trained in CPR and Pediatric First Aid when children are at the facility. LPA reviewed the sign in and out log for children to observed person responsible for the child signs child in and out and records the time of day. Child’s admission agreement is available for review.

Employees or volunteers at day care center; immunization requirements; records; exemptions: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.

Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement was not met by evidence: LPA was not able to verify immunization for staff 1 and staff 2.
