On 10/11/2023 at 8:45 AM an unannounced Required – 1 Year Inspection was conducted for the above facility by Licensing Program Analysts (LPAs) Katrina Chicote and Joe Katrdzhyan. LPAs announced purpose of visit and initially met with A1 who allowed entry to facility. Licensee, Alma Tatlonghari, met at the facility soon after arrival. Facility is currently licensed for a capacity of 18, per Licensee have 10 enrolled. The program currently operates a Preschool Program serving ages 2 until entry to Kindergarten Monday thru Friday from 7:00 AM to 6:00 PM. Per Licensee, they may provide care on Saturday and Sunday based on the needs of the parents. LPAs were taken on a guided tour of the facility of both indoors and outdoors. LIC 125 was provided at time of inspection. LPAs observed and discussed required postings at time of inspection.
At the initial start of the site visit, there was a total census of six preschoolers present and a total census of one teacher (A2) present. An additional two preschoolers arrived at a later time, totaling eight preschoolers present at time of inspection. The facility is made up of two classrooms: RM 1 (located upon entry) and RM 2 (located to the right rear). Facility also has an Infant Program on site #197493736, at 8:48 AM LPAs did not observe Infant Program has separate entrance from Preschool Program and provided technical assistance.

Beginning at 8:55 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPAs observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPAs observed a variety of age appropriate toys for dramatic play, math and science manipulatives free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. LPAs observed cots stored in RM 2 and additional cots stored in bathroom. Per Licensee, sheets are provided by facility and additional blankets provided by parents and they are taken home weekly for washing. Availability of drinking water was reviewed. LPAs observed personal water bottles available for drinking water with water jugs in refrigerator for refills. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed first aid kit is kept in shelf by the Kitchen. At 9:01 AM, LPAs observed carbon monoxide detector operable at time of inspection and smoke detectors and fire extinguishers are present in the facility. Disaster drill log was not available, LPAs discussed requirement. Licensee stated isolation area for sick children is in corner of RM 1 that will not be used by children. LPAs discussed sick policy and procedures with Licensee at time of inspection. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPAs observed these items stored in high cabinet in Bathroom.

Snack/lunch menus were reviewed and observed posted in Kitchen. LPAs discussed requirement to post menus in a prominent place, Licensee states they text parents individually the menus. LPAs observed a text message to parent from Licensee corroborating statement. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 9:08 AM, food preparation areas were toured for safety, cleanliness and proper equipment. Licensee states they provide AM Snack, Lunch, and PM snack to children in care.

At 9:10 am, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed two outdoor play areas available for children, Outdoor Play Area 1 is outside patio and Outdoor Play Area 2 is steps below patio. Required shade and fencing were inspected. LPAs observed Outdoor Play Area 1 to have brick flooring through out shaded with child size tables and chairs available maintained. At 9:12 AM, LPAs observed a clutter of items stored in corner of this Outdoor Play Area 1 in an accessible area such as boxes, trash can, and unused changing table. LPAs discussed observations with Licensee, and per Licensee, they are in the process of cleaning and moving items around the facility. LPAs observed Outdoor Play Area 2 to have turf grass surface through out that is maintained. LPAs observed a variety of age appropriate toys available for children such as plastic ride along toys, plastic play equipment that were maintained at time of inspection. LPAs advised Licensee that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPAs observed water jug with cups available for children's drinking water. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. At 9:45 AM, LPAs observed individual who identified themselves as Dionicia Zamora (A3) present at the facility. Per A1, individual only provides cleaning services to facility. LPAs discussed requirement that all individuals working in the facility must be cleared and associated. LPAs observed uncleared adult leave facility at time of inspection.

Beginning at 10:30 AM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. At 10:33 AM, LPAs observed CPR card expired on 08/30/2021 for A2. Licensee states does not have a current one and confirms no other adult present has current Pediatric First Aid/CPR. Staff present did not have current AB1207 Mandated Reporter Training certificate at time of inspection. LPAs discussed immunization records requirement for staff at time of inspection.

Incidental Medical Services:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com/

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.



Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.

A notice of site visit was given and must remain posted for 30 days.
