On 10/30/2025 Licensing Program Analyst (LPA) Cristina Castellanos arrived at the above-mentioned address for the purpose of delivering findings of the above-mentioned allegations. LPA was greeted by Licensee Ana Montenegro and toured the facility both indoors and outdoors. LPA observed seven (7) children in care with Licensee Montenegro and assistant (S3) providing care and supervision. At approximately 09:46am two (2) children were dropped off at the home. Present during today's inspection was Licensee Montenegro, licensee’s assistant and nine (9) children in care.

The investigation of the above-mentioned allegations was conducted by LPA Castellanos.

On 08/15/2025 Licensing Program Analyst (LPA) Cristina Castellanos conducted the initial complaint investigation at the above-mentioned facility. (8) children in care with two (2) adult staff members providing care and supervision. Present during today’s
inspection was Licensee Montenegro, and assistant (S2). Shortly after at approximately 10:32am Co-Licensee Maria Castro arrived. LPA obtained and reviewed the following documents: Children's Roster, the Child & Adult Care Food Program (CACFP), and weekly program attendance (07/14/2025 to 08/15/2025). Additionally, LPA Castellanos initiated children and staff interviews.

Based on the investigation conducted, interviews of all relevant parties and record review, there is not enough information to prove or disprove that the provider isolates children in a room that is not a comfortable temperature, that provider is not providing children with water, and that provider does not treat children with dignity and respect. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted, and report was reviewed with Licensee Ana Montenegro. A copy of this report and appeal rights were discussed and left with Licensee Montenegro. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
