On June 17, 2021 at 10:38AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an unannounced Annual/Random inspection. LPA disclosed the purpose of inspection and was granted entry by Director Karen Scovell. This is a combination center that also has a preschool component (197493363). The center’s hours of operation are 6:30AM to 6:00PM, Monday through Friday. Infants are located in the rear of the center physically separate from the other component. Upon arrival LPA counted 5 infants and 2 teachers. At approximately 1:00PM, Licensee Lionel Batoba came to the facility. Incidental Medical Services (IMS) policy was discussed.

Upon entry, LPA determined both staff are not on association list. Staff 1 is not fingerprint cleared, but has live scan appointment at 4:00PM, today. Staff 1 left facility during inspection and one infant was picked up to maintain ratios. Upon review of Staff 2's file, Staff 2 had a completed fingerprint clearance transfer request. Licensee disclosed they have been having issues with Guardian and had conducted the transfers using the new Guardian system. LPA obtained copy of criminal record clearance transfer request during inspection.

Furniture and equipment were inspected for age appropriateness and good repair high/low chairs and feeding tables have broad based legs, chair trays lift and lock, high/low chairs are washable, changing table padding thickness in regulation and is within arm's length of sink. Each crib is occupied by one infant. LPA observed staff during lunch and nap. Bedding is washed weekly, and changed as necessary more frequently if soiled. The bedding is stored separately and properly inside the cubbies. There is an Infant Daily Report and a Drop Off information (when was last time infant ate/sleep, last diaper change, how long slept, how much ate) sheet for parents to complete. Used diapers are put in trash container with lid (diaper genie). Each child has container labeled (name) with personal items. Infant room is clean and safe.

Telephone service was verified. Heating, lighting, and ventilation are adequate. There are cubbies (name) for children's belongings in the infant room. LPA observed age appropriate toys and materials. Drinking water is available inside the classroom (sippy cups, bottles). LPA observed a sufficient quantity of cribs available for napping children. LPA inspected and observed changing table (within arm reach of a sink), observed soap, toilet paper and paper towels readily available. Water temperature was tested and is appropriate.

Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. There is a cushioned material and concrete area for active play. The area was observed to be free of debris. The facility has maintenance scheduled to come to facility to install canopy, though shade is provided by building. Due to inclement weather, children are not using outdoor space for play until after heat wave has ceased. Director submitted declaration. Drinking water is available. Staff inspect outside play area prior to children exiting to the play area. Play area was inspected for hazards and inaccessibility to bodies of water. Outside area is completely fenced. The infant play area is separated from the preschool area by a fence/gate.

Children are inspected for illnesses as they arrive. As a COVID-19 safety precaution, all children temperatures are taken, staff conduct an overall wellness check, and staff complete symptom questionnaire. A review of medication policy indicated that prescription medication is administered only with parent's written permission. The Director administers medication and documents the dosage, date and time onto a log. Medication can be brought and taken home by the parent daily. Medication is properly labeled and stored in its original container. There is a separate area for isolation and care of ill children in the Director office.

There is a kitchen area with refrigerator (kept locked) in infant room. Full kitchen in back of center with oven, microwave, refrigerator, and freezer. Chemicals (locked) and food are kept in separate cabinets. Parent can bring breast milk (stored in the

kitchen and labelled)--currently no parents bring breast milk. The center provides breakfast, lunch and afternoon snack (baby food meals). Menus are posted in the lobby, kitchen, and outside the classroom. Allergy lists are posted in kitchen. LPA observed an appropriate amount of food and snacks for infants during the inspection.

Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Infant file has Needs and Services Plan. The Individual Sleep Plan (LIC9227) is missing from the infant files. Staff verbally complete safe sleep observations in 15 minute intervals, but do not have written record. Director advised a written record is required. Personal rights of children were discussed. Children's records and staff records were reviewed.

Sign in and out sheets were reviewed. LPA observed Licensing and Parent boards to be incompliance including the required posting (menu, daily schedule, community activities, licensing required documents). Fire/earthquake drills current. Roster current and maintained. First Aid Kit observed. CPR/First Aid expire 07/20/2021. Note: The training must be taken through American Red Cross, American Heart Association, or through Emergency Medical Services (EMS) Authority approved trainers. Online courses are not acceptable and would result in a citation.

A Civil Penalty of $1000 has been assessed during this inspection for staff criminal record clearance. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.
Licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. A copy of this licensing report (LIC809) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent. A copy of the Acknowledgment of receipt of licensing report (LIC9224) was
provided and must be kept in each child's file. In addition, any child enrolled within the following 12 months must also receive a copy of the Type A Citation.

The On Duty Worker is available for questions at 661-202-3318 Monday through Friday 8am-5pm. An exit interview was conducted, a copy of this Report, Appeal Rights, and Notice of Site Visit was provided to Director.
