On June 23, 2022 at 9:20AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka conducted an unannounced Annual/Random inspection. LPA disclosed the purpose of inspection and was granted entry by Director Karen Scovell. This is a combination center that also has a preschool component (#197493363). The center’s hours of operation are 6:00AM to 6:00PM, Monday through Friday. Infants are located in the rear of the center physically separate from the other component. Upon arrival LPA counted 2 infants, 5 toddlers, and 3 teachers. Incidental Medical Services (IMS) policy was discussed.

Furniture and equipment were inspected for age appropriateness and good repair high chairs and feeding tables have broad based legs, chair trays lift and lock, high chairs are washable, changing table padding thickness in regulation and is within arm's length of sink. Potty training is not done for this age group , but is done for the "2s Classroom" in the preschool component. LPA observed infants during nap and free play. Bedding is changed daily for the younger infants and bedding for the older infants are returned to their cubby, washed weekly. The bedding is stored separately and properly inside the cubbies. There is an Infant Daily Report identifying the infant's sleep patterns, feeding, and diapering. Used diapers are put in trash container with lid. Each child has a container labeled with personal items. Infant room is clean and safe. There are 3 cribs utilized for infants. LPA observed cribs to adhere to Safe Sleep regulations. Cribs without blankets, toys, or pillows inside crib. No items observed hanging from the side or above the crib. LPA observed one infant (9 months) asleep in crib with LIC9227 updated in file.

Telephone service was verified. Phones are available in each classroom for staff use. Heating, lighting, and ventilation are adequate. There are cubbies for children's belongings in the infant room, labeled with the infant's names. LPA observed age appropriate toys and materials.

Drinking water is available inside both classrooms (sippy cups, bottles, and disposable cups with water pitcher). Toddler component kept separate from infant and preschool children. Toddlers share a classroom with preschool children, but maintained separate by partial wall. LPA observed a sufficient quantity of cots and cribs available for napping children. LPA inspected and observed changing table (within arm reach of a sink), observed soap, toilet paper and paper towels readily available.

Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Facility has outdoor play schedule for toddlers and preschool age children. Infants have a separate play area with shade gated all around. There is outdoor playground cushion beneath play structures and in infant play area. The area was observed to be free of debris. There is an area for shade and rest under a canopy. Drinking water is available to children with water pitcher and sippy cups. Staff inspect outside play area prior to children exiting to the play area. No pool, spa, or permanent body of water on premises. During inspection, LPA observed children playing in small wading pool and engaging in water play with sprinkler with direct supervision by staff. Outside area is completely fenced.

Children are inspected for illnesses (wellness policy) as they arrive. A review of medication policy indicated that prescription medication is administered only with parent's written permission (licensing medication form- LIC9221 - also used). The Director administers medication and documents the dosage, date and time onto a log. Medication can be brought and taken home by the parent daily. Medication is properly labeled and stored in its original container. There is a separate area for isolation and care of ill children in Director office.

There is a clean fully equipped kitchen (off limits) with refrigerator, freezer, stove and microwave oven. The facility provides breakfast, lunch, snacks (afternoon, evening). Allergy lists are posted in the kitchen and classrooms. There is a refrigerator and microwave in infant classroom, kept clean. Chemicals and food are kept separate inaccessible to children. Parent can bring breast milk (stored in the kitchen and labelled)--currently no parent bringing breast milk. Menus are posted in the lobby, classroom, and kitchen. LPA observed an appropriate amount of food and snacks for infants/toddlers during the inspection.

Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Infant files have Needs and Services Plan/Individual Sleep Plan. LPA observed the Infant Sleep Plan sections C and D incomplete for Child 1 and Child 3 files. Sleep chart documenting child safe sleep patterns in 15 minute intervals is complete. Personal rights of children were discussed. Children's records were reviewed and LPA observed Child 2's file missing the Physician Report (LIC701). Staff records were reviewed and LPA observed immunization records missing for all staff files reviewed. Staff 1's file is missing the Health Screening Report (LIC503) and Mandated Reporter training expired 12/03/2021.

Sign in and out sheets were reviewed. LPA observed Licensing and Parent boards to be incompliance including the required posting. Fire/earthquake drills current. Roster current and maintained. First Aid Kit observed. CPR/First Aid expire 07/03/2023.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Center Director advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation (call within 24 hours and submit written report within 7 days) and on the form LIC624. Center reminded to review updates/regulations as well as quarterly updates on Department website. The "Notification of Parent's Rights" poster must be posted in an area of the facility accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.

California Code of Regulations, Title 22, 101430 Infant Care Activities, 101216 Personnel Requirements, and 101220 Child's Medical Assessments Type B deficiencies were cited during this inspection.
