On 7/25/19, Licensing Program Analyst (LPA) Helen Estrella conducted an unannounced case management inspection to the facility in conjunction to a complaint investigation. Upon arrival, LPA met with the Director Elaine Wikkramatilleke and informed the nature of the visit. LPA confirmed with the Director that all staff have obtained a Criminal Record Clearance and/or exemption and are associated to the facility. LPA was guided on a tour of the facility (inside and outside).

The facility consists of 4 classrooms and the following teacher to child ratios were observed as follows:
  • Dolphins Class: 8 children being supervised by 2 staff
  • Sea Turtles Class: 12 children being supervised by 3 staff.
  • Star Stars Class: 12 children being supervised by 2 staff
  • Sand Dollar Class: 11 children being supervised by 2 staff.

During the course of the complaint investigation on 7/25/19, it appeared that Child #1 was able to gain partial access to the outside of the facility via the outdoor yard gate. Staff observed the incident and Staff #1 who was outside of the facility at the time, immediately retrieved the child to return to Staff #2 who was inside the outdoor play yard. Although the facility took precautionary measures and placed barriers underneath the gate in the outdoor play yard and informed all staff of the importance of supervision in the center after the incident took place, the facility failed to notify the Department of the Unusual Incident within 24 hours of its occurrence.

The facility is being cited Type "A" for Title 22 Regulation 101212(d)(1)(C) and Title 22 Regulation 101238(a) the attached LIC 809-D page: Reporting Requirements & Buildings & Grounds.

A copy of this report, appeal rights were provided to the Director and an exit interview was conducted.
