Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced annual inspection to the above facility. LPA met with Candida Villegas, Licensee, who guided analyst on a tour of the facility. Also present was Imelda Fabian-Garcia, Co-licensee. Per Licensee, operation hours are 6:00 AM to 6:00 PM Monday -Friday. There are 7 children present upon arrival.

This is a one-story home located in the rear of a duplex consisting of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room, garage.


Areas accessible to children are: living/dining room (main daycare area), kitchen, 1 bedroom in the rear of the home. The children use the bathroom off living room.
Off limit areas include: 2 bedrooms, bathroom and closet in the rear bedroom, garage and front unit.

LPA observed that the doors to off limit areas are locked and/or have knob safety covers. LPA observed a wall heater with a cover. The home has a portable AC unit which maintains the home comfortable. The licensee provides food for children in care. LPA observed children's food labeled with child's name.


Individuals who reside in the home were discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in the licensed childcare home.
All areas identified as accessible for children to use were inspected for safety, comfort, and cleanliness. The restrooms that children use were observed to be safe and sanitary. There is telephone service via a dedicated cell phone. Safe toys, play equipment and materials were observed inside and outdoors..
The valve on the required 2A 10BC fire extinguisher indicates fully charged. Smoke and carbon monoxide detectors were tested and are operable.
Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children. Poisons are locked outside in the locked garage.

Licensee states that there are no firearms stored in the home.

Licensee is currently caring for infants. However, there are no infants 12 months or younger enrolled. Licensee states that infants will nap in the living room in the a playard if needed. LPA advised Licensee that no objects shall be hanging above or attached to the side of the crib. LPA did not observe any infants swaddled while in care. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee.

Currently, children are using the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. No Hazards were observed.

Licensee states that she is not transporting children. Licensee was advised that car seats shall only be used for transportation, never for sleeping children. LPA did not observe any children sleeping in car seats.

Children’s records were reviewed, including emergency information and were observed to contain children's emergency identification forms. During children's file review at 12:37, LPA determined that there are 3 infants in care and a total of 7 children, licensee is found to be over capacity. This poses an immediate risk to the safety of children in care. LPA provided licensee with a Capacity information sheet. Licensee provided LPA with a complete children’s roster.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 07/10/22. There are first aid supplies available. LPA advised that if a child shows signs of illness he/she/they shall be separated from other children.

The licensee does have proof of immunization against influenza, pertussis, and measles. LPA observed that the Licensee does have proof of the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file. LPA issued a Children's Record Review (LIC 857) to the licensee which documents children’s files reviewed during this inspection.


LPA did not observe any pools, spas, hot tubs, fish ponds, or similar bodies of water during the inspection.

LPA provided the licensee with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices. Page 2 of 3

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised the licensee to access forms, regulations and quarterly updates online at: www.ccld.ca.gov.


LPA provided and discussed the Provider Information Notices PIN-20-24-CCP Regulation on Safe Sleep, PIN 19-02-CCP, COVID FAQ - PIN 20-11-CCP, Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.
LPA observed Licensee and staff wearing face masks in accordance with COVID-19 Department of Health Guidance.

The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee/Director.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



Exit interview was conducted with Candida Villegas, Licensee, including, but not limited to Appeal Procedures, Site Visit and Initial Appeal Rights.
