A Case Management site visit was conducted today by Licensing Program Analysts (LPA's) Raul Navarro and Elka Chavez to inspect and measure the school age facility for a capacity determination. Applicant is currently Licensed for a school age program with a capacity of 14. Applicant is requesting to increase the capacity to 30. The school program operates from Monday through Friday 6:00AM - 6:00PM.

The fire clearance was granted for 30 school age children on 06/06/2019. LPAs observed 3 toilets and 3 sinks. LPAs measured indoors and outdoors as follows:

Indoor Space:
School Age Classroom- Total Indoor Activity space: 1,079.725 / 35 = 30 children

Total Indoor Space allows for 30 children. Sufficient napping equipment was observed.

Outdoor activity space:
Front area: 1,202.613 / 75 = 16 children
Preschool Area: 9,183.215 / 75 = 122 Children
Total Outdoor Activity Space: 10,385.8211 / 75 = 138 children

Based on the outdoor measurements, the facility can only be licensed for 16 children. Assistant Director Sheriah Smith has requested a waiver for a capacity increase of 30. Director stated the children will use the preschool outdoor activity space in a scattered schedule. Capacity of children using the outdoor play area will not exceed 30 children at one time. Per Assistant Director, facility will make sure that the preschool and school age children do not commingle.

Exit interview was conducted with Assistant Director Sheriah Smith. The Assistant Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
