On 3/15/2021 at 9:00AM, Licensing Program Analyst (LPA) Lillian Casillas conducted an announced Increase In Capacity Pre-Licensing Tele-Inspection via Zoom with Licensee, Natasha Smays. Licensee guided LPA Casillas on a tour of the inside and outside of the Family Child Care Home. Hours of operation are Monday through Sunday, 24 hours. LPA observed four (4) children in care-- two (2) 4 year olds, and two (2) school-age children.

This facility is a single story, 2 bedroom, one and 1 1/2 bathroom home with a living room, dining room, breakfast nook, kitchen, front yard, backyard, and detached garage. Childcare areas are: living room, dining room, bathroom #1, breakfast nook, and driveway area of backyard. Off-limits areas: bedroom #1, bedroom #2, bathroom #2, kitchen, front yard, grassy area of backyard, and detached garage.

Indoor

Parents drop off their children at the front door. LPA observed Center for Disease Control (CDC) announcements posted on the window next to the front door for parents to see. LPA observed a large plastic container for children to leave personal items outside of the house. Licensee takes the children's temperature with a touchless thermometer once children enter the home. LPA observed hand sanitizer and Community Care Licensing (CCL) COVID-19 related posters at the entrance. LPA observed a 2A10:BC fire extinguisher at the entrance, serviced on 2/17/2021. The breakfast nook is used an isolation room if a child feels sick. This room is also used for food storage and contains a water cooler. Licensee and assistant serve water for the children. LPA observed a baby gate separating the entrance from the living room. The living room is large and includes two tables, age-appropriate toys and books, book shelf, cubbies, television, educational games,

chairs and that car seats are utilized only for transportation.

Parent’s Rights Notification. The "Notification of Parent's Rights" (PUB394) was discussed with the licensee and the licensee was advised that it must be posted in an area of the home accessible to parents. Title 22 102419.

Nutrition Requirement. Licensee was made aware of AB 290, commencing January 1, 2016, requiring at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university. Health & Safety Code 1596.8661.

Mandated Reporter Required. Licensee was made aware of AB 1207, commencing January 1, 2018, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: www.mandatedreporterca.com Health & Safety Code 1596.8662.

Licensing Reports Given to Parents Required. Upon receipt by the licensee, licensees are to provide to parents/guardians the following: Copies of any licensing reports that document a Type A citation- this includes facility visits and substantiated complaint investigations; copy of licensing documents pertaining to a conference conducted by a local licensing agency management representative and the licensee of this family child care home in which issues of noncompliance are discussed or copies of a summary of an accusation indicating the Department's intent to revoke the facility's license. Copies of any of the above licensing documents the licensee has received in the prior 12 months shall be provided to parents/guardians of newly enrolled child at the facility. Assembly Bill 633.

Incidental Medical Services Awareness. Incidental Medical Services (IMS) policy was discussed. For IMS

Facilities that provide Incidental Medical Services (IMS) must identify those services in their facility’s Plan of Operation and submit an updated Plan of Operation to the Department. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag.

LPA advised Licensee to follow and remain updated with COVID-19 guidelines.

An exit interview was conducted with Licensee on 3/15/2021 at 10:50AM and a copy of this report (LIC809) was sent to Licensee via email. LPA Casillas requested Licensee to reply to the email in lieu of a signature.

educational posters, CCL COVID-19 related posters, and cots. Licensee stated that younger children eat on the tables, while older children eat on individual table tops. Licensee stated that she provides food for the children. LPA observed a screen to protect children from the wall heater in this room. A baby gate divides the living room from the dining room. Licensee stated that the dining room and living room are separated to keep the older kids in one room while they participate in remote learning, while the younger children play in the other room. In the dining room, LPA observed age-appropriate toys as well as CCL COVID-19 posters and educational posters. Licensee stated that children's records (immunization records, application, emergency contact, parent's rights) are stored in a secure desk in this room. LPA observed all children wearing masks.

The bathroom includes one (1) toilet and one (1) sink. LPA asked Licensee to open the cabinet underneath the sink and observed nothing stored. Licensee stated she does not store items in the cabinet. LPA observed CCL COVID-19 hand washing posters in this room. Licensee stated only one (1) child is allowed in the bathroom at a time.

Off-Limits Indoors


LPA observed a baby gate separating the dining room from the hallway, which leads to the bedrooms and bathroom #2. LPA observed a functioning smoke/carbon monoxide detector in the hallway. Bedroom #1 is Licensee's bedroom and bedroom #2 is Licensee's daughter's bedroom. Bathroom #2 includes one (1) toilet, one (1) bath, and one (1) shower.

In the kitchen, LPA observed baby gates separating it from childcare areas of the home. LPA observed disposable plates and utensils, per CCL's COVID-19 prevention guidelines. LPA observed a clean and organized refrigerator. While this area is off-limits, LPA observed child safety locks on cabinet doors.

Outdoor
The door to the backyard features two locks--one door knob lock and another lock at the top. The backyard area is divided into two spaces--the driveway and the grassy area--by a wooden gate. Children play on the driveway area. LPA observed three (3) tables, toy cars, tricycles, scooters, chalk boards, and a children's toy

Off-Limits Outdoors
LPA observed a garage in the backyard. Licensee stated that the garage door is made inaccessible to children with a padlock. LPA observed that the garage is used as storage area. The grassy area of the backyard is off-limits due to paint chips that were observed in the facility's Pre-Licensing Inspection on 12/20/2017. LPA observed a wooden gate to make this area inaccessible from the driveway area where childcare is conducted.

Licensee states there are no firearms or weapons of any kind in the facility at the time of the visit. Upon review of child files, LPA observed all required documents according to Title 22 Regulations. On the parent board, LPA observed license, Emergency Disaster Plan, and Notification of Family Child Care (PUB394). LPA also observed a First-Aid Kit located in the breakfast nook.

Discussed topics:
Baby Walkers/etc Prohibited. Licensee was made aware that state law prohibits baby walkers, bouncy seats, exersaucersand any other items that fall into that category. Health & Safety Code 1596.846(b) and (c).

Smoking Prohibited. Licensee was advised that regulation prohibits the smoking of tobacco in a private residence licensed as a family child care home during the hours of operation. Health & Safety Code 1596.795(a).

SIDS & Shaken Baby Syndrome Awareness. Licensee was reminded that all infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome), and to prevent the Shaken Baby Syndrome, Never shake a baby!

High Chair & Car Seat Awareness. Licensee was also reminded that only children eating may be in high
