Licensing Program Analyst (LPA), Keyona Scott, conducted an unannounced annual required inspection to the child care center on 10/20/2022. Upon arrival, LPA met with Director, Cindy Salaues, at 12:45 PM. Health and Safety Director (S1), was also present during the inspection. LPA was guided on a tour inside and outside of the child care center. LPA arrived at facility during nap time. While conducting census of children in care, LPA observed some children napping and others resting quietly on their napping cots. LPA observed proper teacher to children ratios as follows: Orange classroom (4 years to 5 year old) ten (10) children to one (1) teacher; Yellow classroom (2 years and 9 months to 4 years old) five (5) children to one (1) teacher and Green classroom (2 years to 2 years 9 months year old) eight (8) children to one (1) teacher. All adults, working or volunteering at facility have a criminal record clearance or exemption.

The preschool program serves children ages two (2) years to five (5) years old. The operation hours are Monday through Friday 8:00 AM to 5:00 PM.

PHYSICAL PLANT
All areas identified on the Facility Sketch were inspected. There are five classrooms accessible to the children in care: Orange (4 years to 5 years old), Yellow (2 years 9 months to 4 years old), Green (2 years to 2 years 9 months year old), Purple (movement room) and Blue classrooms. The blue classroom is currently closed and does not service any children at this time. The purple classroom is used as the movement room in which gymnasium activities are performed. LPA observed mats, water dispenser, tunnel play sets, hula hoops and other age-appropriate activity playthings in the purple room. LPA observed the following in each of the three active preschool classrooms: water dispensers, outlet covers over the electrical outlets, table and chair sets, kitchen play sets, easels, books, blocks and other age-appropriate playthings. LPA also observed napping cots in each classroom. Per Director, children bring own water bottle from home for drinking water or if child's water bottle is not available a cup is provided.

The facility was kept clean, neat and orderly. LPA observed three restrooms, accessible to the children in care. The restrooms located in the Orange and Yellow classrooms each have two toilets, one sink and changing table(s). The restroom located in the Green classroom has one toilet and one sink. The changing table in the Green classroom is located outside of the restroom door. LPA observed toilets and sinks to be safe, sanitary and functioning. All poisons and hazardous items are stored and inaccessible to children in care. All floors are clean and safe. LPA observed the classrooms and all kitchen areas and/or food preparation areas to be clean and free of litter, rubbish, rodents and/or any other vermin, flies, gnats and other insects. Trash cans used to discard food have tight fitting lids.

Outdoor play area is located at the rear of the facility. The play apparatus is certified approved for children ages two (2) to five (5) years old. Outdoor play equipment is in safe condition and is free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. The outdoor play area is fully cushioned to absorb a fall. LPA observed age-appropriate play structures and equipment in the outdoor play area. There is adequate shade area in the outdoor play area. Drinking water is readily available both indoors and outdoors.

FACILITY RECORDS

There is at least one person present at facility that is trained in Pediatric CPR and First Aid. Per file review, LPA observed completion of Mandated Reporter training certificate in staff's files reviewed.



LPA reviewed four (4) staff files of the following personnel records during today's inspection: Staff Qualifications; Proof of immunization of measles (MMR), pertussis (Tdap) and influenza; Current Pediatric CPR and First Aid Certification; TB clearance or risk assessment; LIC 503 (Health Screening Report); LIC 508 (Criminal Record Statement); LIC 9108 (Statement Acknowledging Requirement to Report Child Abuse); Mandated Reporter Training Certificate; LIC 9052 (Employee Rights)

LPA reviewed six (6) children's files of the following children's records: LIC 613A (Personal Rights); Admission Agreement; LIC 701 (Physician's Report); LIC 995 (Notification of Parents' Rights); LIC 627 (Consent for Emergency Medical Treatment); Immunization Record

  • Verification of Disaster Fire Drills: Per review, last fire and earthquake drills were conducted on 10/14/2022 with seven (7) staff and 25 children in attendance.

  • Sign In/Out Sheets: available for parents to complete electronically through app or manually in-person at the door upon entrance inside the facility.

LPA identified the following locations during today's inspection:
  • Medication Storage Area for Children Requiring Permissible Medical Services: LPA observed medications to be properly stored and inaccessible to children in care

The following was thoroughly discussed:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

The facility was operating in substantial compliance during today’s inspection on 10/20/2022. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies cited.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Director, Cindy Salaues.
