13 | On 4/21/2023, Licensing Program Analyst (LPA), Loyce Phillips, conducted an unannounced visit for the purpose of delivering the findings on the above allegations. LPA was greeted by Director Emily Walton. LPA toured the facility and observed 49 children in care with 10 staff.
LPA conducted the initial inspection on 2/23/2023 and secondary inspection on 3/1/2023. During both inspections, LPA observed adult scissors accessible to children in care in 2 different classrooms. Based on observations and interview statements from Staff #3, #5 and #6 it has been determined that staff left dangerous item accessible to day care children. Therefore, the allegation is deemed Substantiated. Substantiated findings mean that the allegation is valid because the preponderance of the evidence standard has been met.
Deficiencies are being cited accordance to Title 22 of the California Code of Regulations and/or Health & Safety Codes. An exit interview was conducted, a copy of this report, appeals rights and a notice of site visit were discussed and provided to Director, |
