25 | Licensing Program Analyst (LPA) Denise Gibbs conducted an unannounced case management inspection to the above facility on 2/10/20 on 9:00 AM. LPA met with Alma Luna, Licensee, who guided analyst on a tour of the facility. The purpose of this inspection is due to licensee’s request for a capacity increase on 8/19/19. Currently, the licensee’s capacity is eight. Licensee wishes to increase to a capacity of fourteen. Per Licensee, family members residing in the home are 4 adults and 1 child. Licensee's fiancé has 2 children that live in the home occasionally throughout the year. Per licensee, operation hours will be Monday to Friday 6:00AM to 8:00PM. There was one child present upon arrival. Two more children arrived shortly after. Also present during inspection was S1 and A1. A current children’s roster was available for review.
All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a one-story home which consists of 4 bedrooms, 2 bathrooms, kitchen, dining room, living room, front yard and backyard (fenced). This home has 2 properties on the lot. The back house has a different address, 606 W. 42nd St.
Per Licensee, main care areas include the living room, front bedroom and dining room. The children use the bathroom in the back hallway behind the kitchen. Areas that will be used by children were inspected for safety, comfort, cleanliness, telephone service (cell phone), ventilation and heating (ceiling fans and wall heater). LPA observed that there is a fireplace and wall heater in the living room, both are barricaded. Detergents, cleaning compounds, medicines, sharp objects and hazardous items that can pose a danger to children are inaccessible in areas designated for children. The Licensee states that there are no poisons at the facility. The Licensee was advised that any poisons must be locked with a key or combination lock. The valve on the 2A10BC fire extinguisher indicates fully charged, as indicated on receipt submitted or service tag observed. Smoke and carbon monoxide detectors were tested and are operable.
