On 08/18/2022, LPA Risher conducted a subsequent visit to attempt to interview additional children. LPA Risher conducted a follow-up interview with Staff 1.
Staff observed child #1 trip and fall forward while playing in the outside play area. Staff rendered aid after they observed child #1 fall and hit her face on a chair. More than one staff were outside while the children were playing in the play area.
Child #1’s 2 front teeth came out after the fall. Staff rinsed child #1’s mouth out and put gauze to help stop the bleeding. Staff had the child sit down after the incident.
Children take naps at the facility after lunch. Staff stated a child can sit up and color if they do not want to take a nap. Staff will have the children lay quietly as well. Children stated they can sit and be quiet if they do not want to take a nap. Facility utilizes cots for naptime. Infants use cribs during naptime.
LPA observed infant chairs in the facility. Staff stated the infant chairs are used for eating only. Staff speak to the children about their behavior. Staff redirect the children as well. Children stated staff tell their parents about their behavior.
A full investigation was conducted which included observations and interviews. The information received did not reveal evidence that neglect/lack of supervision, medication and personal rights violations occurred. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the above alleged violations did or did not occur, therefore the allegations are found to be unsubstantiated. Staff observed child #1 get injured and rendered aid. Children can sit quietly or participate in an alternative activity if they do not want to take a nap. Facility utilizes redirecting and speaks to the children about their behavior.
Exit interview was conducted and report was provided to Toni Hayes, Licensee. Appeal rights will be provided.
