Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 3 year inspection. LPA met with Ashley Castro, Director, who guided analyst on a tour of the facility. This is a school age program serving up to 115 children. The facility operates Monday, Wednesday, Thursday, and Friday from 3:00 PM to 6:00 PM, and Tuesdays from 1:30 PM to 6:00 PM. All children are under supervision, including visual supervision of a teacher at all times. A review of the sign in/out sheets was conducted to verify the current census of children. Facility is operating within capacity limitations. The facility consists of 4 classrooms (1, 2, 5, and 18). This center is located on the grounds of Colfax Charter Elementary School. All four classrooms are part of the school operated under the LAUSD and available to the Peak Program from 3:00 PM - 6:00 PM. The facility was licensed for Classrooms 1, 7 , and 19 and has fire clearance for these classrooms as well. However, per the director, this school year Colfax Charter Elementary School provided the classrooms they have available which can change each year. This year they are utilizing classrooms 1, 2, 5, and 18. The classroom changes has not been reported to licensing. LPA observed 24 children in Room #2, supervised by two staff, 12 children in Room #1 supervised by one staff, eight children in Room #5 supervised by one staff, and 14 children in Room #18 supervised by one staff. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. There are no bodies of water on the premises during the visit.
LPA observed the following postings by the Director's office (Room #103: License, LIC 613A, the PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems (PUB269).

Furniture and equipment were inspected for age appropriateness and good repair. A walk through of the classrooms was conducted, classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. Classroom floors are clean and safe.

There is a restroom in the classroom #1 with one sink and two toilets. The children also utilize the restrooms on campus, girls and boys have separate restrooms. Age-appropriate sinks and toilets were inspected for availability and good repair in all restrooms. Per Director, the isolation area is located in the office.


General sanitation was observed. Children bring their own water bottle and before entering the classroom and before going outside, the children fill their water bottles with water from the containers in each classroom. LPA did not observe during file review a lead test for the center.
Disinfectants and cleaning solutions were accessible to children. Carbon monoxide detectors were observed in each classroom and is operable.

The center does not have a kitchen and does not prepare snacks for the children. The children bring snacks from home. First Aid supplies were observed. Per Director, medication is not administered at the facility.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

LPA conducted a Children's Records Review for completeness and to ensure children have proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights on file. The children's roster was available and current. Staff Records were reviewed to ensure the following: Criminal record clearance or criminal record exemption, education credits required, current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination, Mandated Reporter training. Four staff do not have current Mandated Reporter Training Certificates and two staff do not have proof of education credits, Director doesn't have current CPR/First Aid in files. Page 2

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
