25 | Licensing Program Analyst (LPA), Veronica Wheatley conducted an unannounced inspection and meet with Teacher Tomi Aoyama and Teacher James Aoyama at 4:35PM. The director Yoshio Aoyama is not present. They stated he is at the other location today. Upon arrival, LPA observed 11 children (infants) inside of the first classroom with Teacher Tomi Aoyama and Staff #2. Due to limited English, she called her son who is on the premises to translate. LPA informed Tomi and James Aoyama that the facility is out of ratio. The ratio for infants is 1:4. (One qualified teacher with 4 infants). This is a repeat Title 22 Regulation violation from a previous occurrence which occurred on August 17, 2021 and will result in a civil penalty.
LPA also informed Tomi and James Aoyama that the facility is operating over the maximum licensed capacity which 10 children/infants.
LPA asked for the children's names that were present. While documenting the names, LPA observed Child #1 sitting in a stroller in the classroom that was not used for its intended purpose. This is a personal rights violation according to Title 22 Regulation. LPA received a children's roster with the names of the 11 children present today.
Upon receipt of a citation for a Type A Deficiency, the director shall post the licensing report for 30 days in addition to the Notice of Site Visit & provide copies of the licensing report (LIC 809D) to parents/guardians of children in care at the facility by the close of business the following day or the next day child returns to the facility. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file.
Exit interview. The report was not signed and provided to the licensee due to the licensee’s son James Aoyama displaying hostile behavior and recorded the LPA without consent. A copy of this report will be mailed certified and via email.. |
