On 12/12/2023, Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced Annual Required Inspection for the Child Care Center mentioned above. LPA Castellanos met with Administrators Tomi Aoyama and James Aoyama. LPA toured the facility indoors and outdoors and observed 5 children in care, being supervised and cared for appropriately by 2 fingerprint cleared staff members. Facility hours of operation are 7:30am – 6:30pm. Facility provides lunch and am/pm snacks as well as water.

Capacity and limitations as specified on the license are being maintained.

All children are under supervision, including visual supervision, of a teacher at all times. The CCC is in compliance with Title 22 regulations regarding Staff to Student ratios. Parents sign their child in/out of the facility. LPA Castellanos advised administrators to ensure that parents sign using their full legal name and the time of day.

There are no swimming pools, ponds, or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons chemicals are kept above the kitchen sink as well as a top shelf in the children's restroom.

LPA Castellanos observed two (2) fully charged fire extinguishers throughout the facility. LPA observed 2 working carbon monoxide detectors. The smoke detectors and carbon monoxided are wired to the Pull Fire Alarm system recently installed in June 2023.

All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. LPA Castellanos inspected the classroom and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Activity play equipment is in safe condition, free of sharp, loose, or pointed parts.

LPA Castellanos conducted a Health and Safety inspection of the kitchen, and all kitchen food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair.

LPA Castellanos reviewed 5 children’s files and observed files to be in compliance including current contact information for authorized representatives and/or relatives who can assume responsibility for the child, immunization records, and authorization for medical treatment.

LPA Castellanos reviewed 4 personnel files and observed files to be in compliance with current CPR/First Aid, Mandated Reporter training and immunizations for personnel in childcare.

Community Care Licensing was notified that the lead water testing conducted at the facility on 10/30/2023 did not exceed the allowable limit for lead 5.5 ppb/ug, the samples tested met regulations. LPA Castellanos verified that the lead testing was completed in accordance with Written Directives outlined in PIN 21-21.1-CCP.

Safe Sleep regulations were discussed with all staff members present during today’s inspection. There is one crib or play yard for each infant in care, cribs and play yards are kept free from all loose articles and objects while infants are sleeping, and there are no objects hanging above or attached to the crib or play yard. Infants are not swaddled while in care. Staff members physically check on sleeping infants every fifteen minutes however do not document any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Infants up to 12 months of age are placed on their backs for sleeping. Individual Infant Sleeping Plan was discussed and LIC 9227 form reviewed with administrators Tomi and James Aomaya.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Administrators were reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

This facility does not provide Incidental Medical Services - IMS. LPA Castellanos reviewed storage of medication and equipment/supplies, and reviewed children's, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Administrators were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA Castellanos reminded administrators to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, there was one (1) deficiency cited at this time for violation of Title 22 regulation; (see LIC809-D).

An exit interview was conducted, and Plans of Correction were reviewed and developed with Administrators. A copy of this report and appeal rights were discussed and left with administrators Tomi Aoyama and James Aoyama. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
