On 6/22/2023, Licensing Program Analyst (LPA), V. Wheatley conducted an unannounced Annual Required Inspection for the infant / toddler license. LPA met with Lead Site Supervisor Carlos Bello and RSD (Regional Site Director) Monique Randolph, and toured the facility indoors and outdoors. LPA observed 11 children/toddlers only. No children under 24 months on the premises. There are two classrooms identified as Classroom 02 and Classroom 01. LPA observed 6 children in class 02 and 5 children in class 01. The facility is operating beyond the license requirements. The days and hours of operation are Monday through Friday 7:30am to 4:00pm. The children are on the premises 8am to 2:30pm.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. There is a working carbon monoxide detector.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off site activities.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. There is a ratio of one teacher supervising no more than six (6) in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child, medical assessment, individual feeding plan, and Infant Needs and Services Plan. Staff records will contain documentation of meeting qualification requirements. Menus are posted at least one week in advance where an authorized representative can view them. The facility has sufficient age appropriate furniture, and equipment including napping cots, changing tables and feeding chairs.

Each cot is occupied by only one child. Infants are not swaddled while in care. Staff physically checks on sleeping infants under age 2 every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Documentation for infants up to 12 months includes sleeping position if it is other than on their back. Individual Infant Sleeping Plan is completed and in file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping. Staff-infant ratio requirements are being met while infants are sleeping.

Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Based on the LPA's observation and interview with the Regional Site Director Monique Randolph, the licensee did not comply with the section cited above in that there are 11 toddlers on the premises. The license is for 8 infants and 8 toddlers. The facility is operating beyond the requirements of the license. This poses/posed a potential health, safety or personal rights risk to persons in care.

The licensee/director will make sure that the license is being adhered to at all times. The licensee will determine if a new preschool license is required instead of an infant license since there are no infants under 24 months enrolled. The licensee will inform The Department of decision by June 26, 2023. If a new application is going to be submitted to the Department, the application will be submitted by 7/21/23.
