On 9/27/2021 Licensing Program Analysts (LPAs) Judy Laureano and Deborah Lowe arrived at facility at approximately 12:14 p.m. and met with Zia Mojabi, Assistant Director and Nassima Mojabi, Licensee/Director. The purpose of a Case Management – licensee-initiated inspection at the facility is to ensure the health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. Facility is requesting a modification to current Infant Program, adding a Toddler Component without changing the total facility’s capacity of 36.

The facility originally had an open floor plan but has changed to operating in 5 spaces divided by half wall partitions bolted to the floor with L shape brackets. The facility is requesting to decrease the infant capacity to 30 children and adding the toddler component to serve 6 toddlers (18 months to 36 months).

On 6/28/2021, STD 850- Fire Safety Inspection was received in the ESRO. Fire Safety Inspection- STD 850 granted on 06/24/2021.

LPAs were guided on a tour of the facility, both indoors and outdoors. The facility operates Monday to Friday from 8:00 a.m. to 6:00 p.m.


LPAs observed all areas being used for child care. In Room #3 LPAs observed 2 changing tables along the wall near the napping area. Diapers, wipes and or diaper rash ointment were observed to be stored in cubbies clearly labeled for children in program. Director Nassima confirmed that parents bring diapers, wipes and diaper rash ointment if needed. The bathroom that children use is across the diapering area.

The facility is serving meals and snack and parents are required to bring Infant Formula and/or Breast milk. Food preparation area was clean. LPAs advised facility to add a safety latch to the cabinet under the kitchen area sink for added protection. The refrigerator, sink and counter space area was observed and barricaded with a safety gate tied with plastic zip ties attach to a table. Within the kitchen area, LPAs observed 15 highchairs and two children size table for meals.

The facility has a designated crib area for children to nap. LPAs observed 15 cribs in the space using long curtains to separate the area. LPAs advised facility to change curtains currently in use due to the curtains being hazardous to children in care. LPAs observed the curtains bundle on the floor. Placement of cribs or play yards shall not hinder entrance or exit to and from the space where infants are sleeping. LPAs observed 15 children in the nap area with 5 children awake. Facility immediately added two additional staff member to assist with supervision. Room #5 has been designated as the Toddler Classroom and Toddler Nap area. LPAs observed 6 children napping with one staff member supervising. LPAs advised facility to ensure COVID 19 precautions around napping are being followed. LPAs observed only one child needed to be moved in the opposite direction making sure children are not face to face with each other while napping.

LPAs inspected furniture to be age appropriate. The napping equipment were in good condition. Children’s belongings were stored separately. Isolation area is located by the director's office.

LPA observed staff restroom located next to the director’s room office. LPAs observed the space to also have facility’s cleaning products, hand sanitizers and extra First Aid products in a locked cabinet.



Per COVID-19 recommendations, sign in and sign out sheets were observed to be located outside the facility for parents to sign. LPAs observed first name used only for child and parent.

Serviced fire extinguishers, smoke detectors/carbon monoxide detectors were observed. The center has daily activity schedule and snack menu posted. LPAs advised fire extinguishers need to be serviced annually.

LPAs observed the outdoor area and observed the required shade for outdoor yard.

Facility was originally measure with an open floor plan. Due to changes made to the original floor plan, LPAs remeasured facility with the following measurements.

Measurement for the Indoor Classrooms

Room # 1: 18.17 x 16.33 = 133.42 sq ft.;
Room # 2: 10.50 x 14.17= 148.79 sq. ft; ;
Room # 3: 10.42 x 18.0 = 177.93 sq. ft.;
Room # 4: 7.67 x 18.0 = 138.93 sq. ft.;
Room # 5: 8.67 x 18.33 = 158.92 sq. ft.;

Kitchen/Food Prep/Kid’s eating area not currently being used as activity space is measured at 18.5 X 16.92= 313.02
The total measurements for the Toddler Option is measured at 8.67 x 18.33 = 158.92
7.67 x 18.0 = 138.93 sq. ft.;divided by 35 sq. ft. = 6 children.


Measurement for the Play Yard
Play yard #1: (18.42 x 28.33- 46.10 (incumbered space) =475.74
Play yard #2/ Toddler Yard: (12.58 x 22.673- 88.47(incumbered space) =196.72

The facility was in compliance with Title 22 regulations during today's visit. The following Technical Violations were discussed: 1012291(a)(1), 101439.1(g)(1),101.227(a).

The following was discussed:

· A child younger than 18 months of age shall not be moved into the toddler program. A child who is older than 18 months of age shall not be required to be in the toddler program.
· Parents shall give permission for the placement of their children in the toddler program.
· A ratio of six children to each teacher shall be maintained for all children in attendance at the toddler program. An aide who is participating in on-the-job training may be substituted for a teacher when directly supervised by a fully qualified teacher.
· The maximum group size, with two teachers, or one fully qualified teacher and one aide, shall not exceed 12 toddlers.
· The toddler program shall be conducted in areas separate from those used by older or younger children. Plans to alternate use of outdoor play space may be approved to achieve separation.
· All other regulations pertaining to the facility’s Infant Program shall be complied with.

Exit interview conducted, and report was reviewed with Zia Mojabi and Nassima Mojabi The licensing determination of this application will be reviewed with Licensing Program Manager for final resolution.
