Prior to entrance to the facility, LPA Mayra Rivera conducted a Covid 19 assessment and based on the licensee responses to the facility assessment questions, LPA Rivera determined safe to proceed. On Tuesday, August 18 2021 at 10:35 AM, Licensing Program Analyst (LPA) Mayra Rivera conducted an unannounced annual inspection and met with Licensee Valjean McCray who guided LPA Rivera on a tour of the facility.

During the inspection, 2 preschool children were present. Family members residing in the home was discussed with licensee and are cleared. Operating hours are Monday to Friday, 6:30 AM to 7:00 PM and care for children ages 0 to 12 years.

This facility is a one-story home that consists of two bedrooms, an add on rhombus room (daycare room), one bathroom, kitchen, living room, front yard and backyard (fenced/gated). Areas that are accessible to children and identified on the facility sketch were inspected by LPA Rivera; living room, kitchen, bathroom, daycare room and backyard. Areas off limits to children include- master bedroom. At 10:45 AM, LPA Rivera observed the master bedroom door closed.

At 10:52 AM LPA Rivera entered the daycare room area to inspect for safety, comfort, cleanliness, ventilation and working phone (land line). For ventilation, LPA Rivera observed portable AC Unit in the daycare room and two wall heaters. One heater is located in the living room and the second heater in the hall way. LPA did not observe a barrier in placed to prevent access to children. LPA advised licensee to place a fire proof screen as a barrier to prevent an injury/burn.

LPA observed children materials such as art supplies, puzzles, and manipulative. LPA observed the furniture and children materials to be in good condition and age appropriate.

At 10:57 AM, LPA Rivera entered the restroom and observed the toilet, running water, hand soap and paper towels. LPA observed the bottom cabinets closed and with a child proof lock in place making it inaccessible to children to open the cabinet doors. LPA observed the restroom to be in good condition.

At 11:00 AM LPA observed cleaning compounds items stored inside the cabinets below the kitchen sink with a child proof lock in place making it inaccessible for children to open. LPA observed kitchen knives stored inside a top kitchen cabinet making it inaccessible for children to reach. LPA observed all kitchen drawers and bottom cabinets with child proof locks in placed making it inaccessible for children to open. Licensee understands that any poisons or firearms must be locked with a key or combination lock. For drinking water, LPA observed water bottles. LPA Rivera asked if there are any pets, poisons, firearms, weapons or bodies of water. Licensee stated she has one turtle. no firearms, weapons nor bodies of water. LPA did not observe firearms or weapons nor bodies of water. Licensee was advised that if any poisons (ex; drano, rat poison or items with skull hazard symbol), firearms and weapons are purchased, it is required to be stored with a lock key or combination lock and firearm and ammunition must be stored separately.


At 11:05 AM LPA Rivera observed the required 2A10BC fire extinguisher located in the daycare room and the valve on the green area indicating fully charged and purchased receipt 8/7/2019. LPA informed licensee the fire extinguisher needs to either be serviced annually or purchase a new one yearly. At 11:07 AM LPA pressed on the combo smoke/carbon monoxide alarm located in the daycare room and heard the sound of the alarm and observed the green flashing light on. The alarm is not a kiddie brand and is operating. The combo smoke/carbon monoxide alarm located in bedroom #2 is a kiddie brand and LPA informed licensee that there's a recall with the brand kiddie on the combo smoke/carbon monoxide alarms.

At 11:20 AM LPA inspected the outdoor area used by children in care for safety, comfort and cleanliness, LPA observed backyard to be fenced all around. LPA observed on the ground a section of the chain link metal fence. LPA advised to remove the chain link metal fence and place it in an area where children don't have access to prevent an injury. LPA observed a shed. The shed is used for storage. LPA observed the shed to be closed and a padlock in place to make inaccessible to children to open. The small metal fence placed on the side end of the shed, LPA observed a section of the metal fence sticking out . LPA advised to remove or place a barrier to prevent an injury. LPA observed the side gates closed and locked with a padlock. LPA observed an EZ Up anchored to the ground with sandbags and provides an area for shade. LPA observed a toddler slide, balls, trick bikes and a hexagon play structure. LPA advised to place a surfacing protection around the hexagon play structure to provide a fall protection. LPA observed play equipment to be in good condition and age appropriate.

At 11::35 AM LPA observed licensee pediatric First Aid/CPR certification (expires 9/4/ 2022) and has proof of immunization against pertussis, MMR and LPA received influenza declination dated 8/18/2021. Licensee has completed the preventative health and safety training on 9/20/18, lead component on 6/1/21 and mandated reporter (AB 1207) training on 3/11/2020. Licensee was advised that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

LPA observed the children's facility roaster, and on the bulletin wall LIC 610 Emergency Disaster plan, Parents Rights, License, facility sketch and earthquake/fire drill log and last drill conducted on 7/8/2021.


The following was discussed
INFANT CARE: LPA Rivera reviewed and discussed Safe Sleeping Practices information with licensee and a plan for supervising sleeping infants. LPA advised licensee to sleep infants in an area where infants can be directly supervised. Place infants on their backs to sleep, on a firm mattress with fitted sheet that fits snugly in crib or play yard. LPA also advised against sleeping infants in a separate room with no direct supervision and no pillows, crib bumpers, swaddling, head covered, and pacifiers must not have anything attached. Safe sleep brochure was provided.

LPA Rivera also reviewed Sudden Infant Death Syndrome (SIDS), Never Shake A Baby, and Lead Exposure information with licensee. LPAs also explained to licensee that car seat, stroller are only and only for transportation, highchair is only and only for feeding and stated items cannot be misused. No smoking, No infant walkers, No Johnny jumpers, No saucer chairs, No trampolines and any other item that falls into this category are not permitted in a family child care facility.



Medication: Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. LPAs advised the licensee how to access forms, regulations and quarterly updates , and Providers Information Notices (PIN) online at: www.ccld.ca.gov

LPA issued the Children's Record Review (LIC 857) to licensee during the inspection. Children's and licensee files are up to date.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



During the inspection, licensee removed the chain link metal fence from the backyard and placed it over the other side of the fence. The small metal fence placed on the side end of the shed, licensee placed it underneath the shed to prevent an injury.

Licensee has been given a technical violation for not having the required 2A10BC fire extinguisher serviced annually or purchased within a year. Exit interview was conducted and plans of correction were reviewed and developed with Licensee Valjean McCray. Appeal rights explained & provided. A copy of this report and appeal rights were discussed and left with licensee Valjean McCray, whose signature on this form confirms receipt of these documents.
