On 1/24/2020 at 9:45AM Licensing Program Analysts Jeanette Estrada, Dalicia Adkins, Claudia Escobedo and Antonio Almanza conducted a Pre – licensing visit at Future Stars Academy located at 4946 West 20th Street Los Angeles, Ca 90016. This facility is for a preschool age program and it is part of four facilities on the premises which are all under the name Future Stars Academy. For this license number, the applicant, Ocean Sky- A California Corporation, is requesting a license of 30 preschool children and will serve children ages 2- 6 years old. Future Stars Academy will be operating Monday- Friday from 6:30 AM – 6:00 PM. LPAs met with Administrators/ owners, Irene Olshansky and Marina Olshansky, who guided LPAs on a tour of the facility. Fire inspection was submitted and was granted on 10/1/2019.

This facility is accessed through an entrance on 20th street. There is a wrought iron gate surrounding the premises which gives parents the option of driving into the facility for morning drop off. Parents will also be able to park on the street and walk children into the facility. The preschool entrance is secured with a key pad lock and the facility administrators will give parents the code to enter the facility. Administrators also stated they would change the code every 3-4 months and notify parents.

Morning drop off is going to take place at this location for the entire school (all 3 programs). Once there are a couple of children from each program and a teacher from each program is also present, they will then disperse into their respective facilities. Sign in sheets for each program will be placed on a table next to the entrance and there are cubbies for mail for the parents to receive. The main office for the four licenses is connected to this building (4946 W. 20th Street). It can be accessed through the 20th street entrance or through the classroom. PG. 1

The program will utilize this entire building, which is consists of an open floor plan, for two classrooms (Classroom A and Classroom B which are separated with a small bookshelf). LPA observed appropriate chairs, tables, and children's equipment. There is adequate heating, lighting and ventilation of the classrooms. There is adequate storage for the children's belongings. Classrooms are equipped with restrooms and sinks for hand washing. All classrooms are equipped with individual cubbies for storage. Drinking water is readily available (via water fountain) for children in care. LPA observed Fire Extinguisher (2A10BC) which was last serviced on 5/29/19, Smoke/CO Alarms, First Aid supplies in classroom and office and there is an emergency supply bin in the back of the classroom.

This program will be for children ages 4-6 years old and it will run two classrooms; Pre-k and K. Since this building is an open floor plan, there is shared space between the two classrooms. Each classroom will use the activity centers in the classroom at different times. This building has 6 large windows along the carpeted area that are kept locked.

To the left of the entrance to this building there is a kitchen, a staff bathroom, a children’s bathroom and a water fountain. The kitchen is accessed through a door that is locked. The facility will provide breakfast, lunch and a snack that will be prepared in the kitchen. Children who will not use the food provided at the facility will be able to have their food stored in the refrigerator in the kitchen. These children’s lunches will be labeled. A fire extinguisher was observed in the kitchen as well. There is also a list of allergies posted on the refrigerator. Cleaning supplies are kept in the kitchen in a cabinet which is latched and inaccessible to children in care. Administrators stated that the kitchen staff will be mainly responsible for cleaning the facility after lunch time, throughout the day and at the end of the day.

From the open space in the classroom, there is a door that leads into the children’s restroom. On the opposite side of the children’s restroom there is an open door-way which leads to the sink in the hallway. This sink is in a trough style and it has 3 faucets, 2 soap dispensers and a napkin dispenser. Next to the trough is a pantry which houses items such as art supplies, paper goods and garbage bags. This pantry is kept locked. Next to the pantry there is a door that leads to kitchen which is also kept locked. In this hallway there is also a doorway that leads to the outdoor area. PG. 2

Nap time will be held in the carpeted area for both classrooms. There are 21 cots in the classroom and the bedding is stored in a bag that is attached to the cot. The administrators stated they are available during nap to step in if needed. The isolation area will be in the director’s main office.

There is enough space in the classrooms for capacity being requested, cumulative indoor: 1052.34 sq ft. allowing for 30 children.

Outdoor equipment was inspected for safety, good repair, age appropriateness and adequate resilient cushioning material underneath. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water.

Outdoor area is accessed through a door in the hallway. There are 6 stairs leading down to the play yard. There is a hand rail on the side of the stairs. There is also a pull-down fire alarm by the door. The outdoor area is covered in artificial turf. There is adequate shade which is held up by posts that are surrounded with cushion. Children have access to drinking water from a water jug. There is a swing set, a wooden house and a metal slide in the outdoor yard. All are securely anchored to the ground. The yard is sloped down and there two sets of small steps leading to the end of the yard. The applicant was advised to place a strip on the steps in order to be seen. There is a lemon tree in the yard with exposed thorns. Applicant was advised to install a mesh netting surrounding the tree and bush area that surrounds the tree. This yard leads to the yards for two of the other facilities on the premises; 197494318 (at the end of the yard) and 197494289 (on the right of the yard). The other yards are kept separated by gates which are inaccessible to children. On the left side of the yard is the infant program which is also separated by a 4 ft. fence.

The total outdoor playground space is 2822.20 sq. ft. which allows for a total of 37 children.

CORRECTIONS TO BE MADE

9 cots need to be made available for a total of 30 which is the requested capacity.

The ledge on the art board needs to be secured or removed.

Lemon tree needs to be made inaccessible to children in care.

Steps in the outdoor yard need to be made easily visible (place a strip of material).

Based on the indoor/outdoor space and number of toilets/sinks, the facility can accommodate the requested capacity of 30 Preschool children. A copy of capacity excel sheet was provided to applicant.



An exit interview was conducted and copy of the report was reviewed and furnished to the applicant. Corrections shall be made to the identified issues by 1/28/2020. Final license determination will be made upon review by the case carrying Licensing Program Analyst and Licensing Program Manager.
The following was discussed:
All postings will be posted at the entrance of the school. Required postings are as follows, Facility License (TBD), Emergency Disaster Plan, Parents Rights Poster, and Personal Rights. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents. The applicant was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224). An appeal may be filed, in writing 15 business days from the date of receiving the penalty assessment.
As of September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.
