On 10/29/2021 at 2:00 pm Licensing Program Analysts (LPA) Deborah Lowe met with site supervisor Mojgan (Sara) Yasharpour. The purpose of the visit was a second announce case management licensee-initiated inspection to review for a capacity change at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no child present during this inspection. This is an application for 140 children, ages 18 months to 6 years old. The child care center will operate Monday – Thursday 8:00 a.m. – 5:00 p.m., Friday 8:00 a.m. – 1:30 p.m. The fire clearance was approved on 09/08/2021.

The 1st pre-licensing visit was conducted on 9/30/2021. LPA Lowe is conducting a 2nd pre-licensing visit to evaluate the facility corrections needed before a capacity increase will be considered.

LPA Lowe evaluated the following corrections need for completion:

  • Outlet cover in room 403 needs repair – LPA observed new outlet cover.
  • Pealing paint rooms 103 and 402 needs repair – LPA observed area with no pealing paint.
  • Hole in wall above fire extinguisher needs repair in room 402 – LPA observed repair made.
  • Bathrooms on 4th floor need cleaning – observed bathroom to be clean
  • Room 303 teacher cabinet make cleaning solutions inaccessible – LPA observed child latch on teacher cabinet.
  • Outdoor 2nd floor climber middle platform is broken and needs repair/ replace – LPA observed a carpeted soft mat attached to climber to cover cracked platform.
  • Outdoor 2nd floor metal cover to top of wall needs repair (is bent with a sharp edge) – observed sharp edge as repaired showing flat to surface.

  • Rooms 103 and 203 broken mirrors on walls need repair/ replace – LPA observed broken mirrors were removed from wall.
  • Air vents in multiple rooms need repair, missing bolts or bent. – LPA observed vents to be secure with missing bolts replaced.
  • Remove strollers from classrooms and outdoor drop off area – LPA observed strollers removed from classroom. LPA observed strollers in outdoor carpool area, site supervisor stated they belong to families.
  • Hallway on fourth floor near room 401 cord hanging from wall needs to be secured – LPA observed cord to be rolled and placed on top of refrigerator inaccessible to children.
  • Room 201 toward ceiling has an squared opening with exposed covered wires, need cover placed. – LPA observed area to be sealed with no exposed wires.
  • Pest control/rodent box needs to be moved and inaccessible to children. – LPA observed the pest control box had been removed from walk way.
  • Room 103 Fire extinguisher needs service. Tag expired 10/10/2020. – LPA Lowe observed new service tag with a service date of 10/06/2021.
  • Provide thermometer for refrigerators. – LPA observed a new thermometer in refrigerator.

LPA Lowe has verified the above needed corrections have been corrected and they are considered completed.

The following corrections are still needed before a capacity increase will be considered.

  • Provide current classroom and outdoor schedules to include room numbers, number of children and staff in each group for waiver. Provide to LPA Lowe no later than 11/03/2021.
  • Updated outdoor waivers will need to be approved by Management.

A notice of site visit was given and must remain posted for 30 days.
