A pre-licensing inspection was conducted today by Licensing Program Analysts (LPA) Shandra Powell and Silva Garibyan, to inspect and measure this facility for a pre-licensing observation and capacity determination . LPAs met with Melanie Wolff and Darien Williams, Applicants and KJ Mayer, Director, who assisted LPA with touring the facility. Survaliance cameras were observed in the facility (Sunset Studios Media Solutions). LPAs observed items posted at entry of facility; Emergency Disaster Plan, Earthquake Preparedness Check list, Parents Rights Poster, Personal Rights, Child Car Seat Law, Menus, Activity Schedule.

The proposed Infant program will operate Sunday through Thursday 7:00am - 7:00pm and Friday through Saturday from 7:00am -10pm. This is a on demand center that will operate 7 days a week. The center is a hourly rate child care center. This center is a combination center (Infant, Preschool and Toddler Components) housed on one site with physical indoor separation for each component. The Infant component has one room with a crib area with 8 cribs. Classroom measurements are as follow: (Area 25.3 X 15.5) - (4.2 X 7.5) = 360.65 /35 = 10 Infants.

The entrance to the facility can be made through Run Way Road shopping center on the first floor. Licensing staff inspected and measured the classroom, there is no outdoor play area for infants. Infants will use the indoor play area which is located in the middle of the facility. LPAs observed applicant to have an operable carbon monoxide detector and smoke detector.



Children will be signed in upon entrance to the lobby where a daily sign-in sheet will be available. Parents will have access to two drop off spots to allow the parents to drop and pick up children. The Director's office is located in the rear of the facility where it will be used as an isolation area for ill children with a napping cot. The staff restroom located in the directors office will be used for ill children. Applicant states that medication will be stored in locked cabinet above refrigerator.

There is age-appropriate furniture and equipment observed in the classrooms which is in good condition. The rooms have central heating and air-conditioning. Individual cubbies are available for children's belongings. One changing table with at least 1 inch thick padding covered with washable vinyl or plastic. Sides raised a minimum of 3 inches. Changing table within arm’s length of sink when in use.

LPAs observed safe toys, with no sharp edges, splinters or points, nor made of small parts that can be pulled off and swallowed. Bedding will be separately identified and stored for each infant. Placement of cribs, cots or mats allows for entry/exit from the napping space. All cribs have identifies located on them for each child's name. Indoor play area was inspected for hazards and inaccessibility to bodies of water.

Breakfast, Lunch, Dinner and Snack will be provided by an outside vendor. All food prep areas have working sinks and a refrigerator. Applicant shall ensure that food is age appropriate and follows Federal Food Guidelines.

This facility plans to provide Incidental Medical Services-IMS. For IMS information, see Evaluator Manual-Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/(800) 514-0383 (TTY) and link publication: Commonly Asked Questions about Child Care Centers and the ADA, available at http://www.ada.gov/childqanda.htm. A New Proposed Safe Sleep Regulations and Best Practices for Safe Sleep was provided.

The Applicant is seeking to provide care for 10 Infants; however, a fire clearance is pending. Applicant resubmitted an LIC 200A with updated information.
