On 4/22/2021 at 9:30 am, Licensing Program Analyst (LPA), Deborah Lowe met with Kenner Rakoz, Director and Melanie Wolff, Owner to conducted a virtual inspection with the purpose of this inspection is to provide technical assistance and ensure the facility is safe for the care and supervision of children in care. Due to COVID-19 state of emergency a Licensed Child Care Center wavier was granted on 4/09/2021 for the facility located at 12746 W. Jefferson Boulevard, Suite 3-3100, Los Angeles, CA 90094 to care for children ages 2 years - 12 years-old due to COVID-19. Hours of operation Monday - Friday 8:00 am -5:00 pm.

Fire clearance for the use of 4 unlicensed rooms on file dated 4/08/2021. The facility will utilize the following classrooms for child care: Staff Work Area, Enrichment Studio, Common Area, and the Adult Work Area.

Check in will take place at the front entrance of the facility. A designated staff member conducts wellness checks for children with temperature checks and screening questions. If children become ill during the day, they will rest in the isolation area located in the director office. Separate isolation restroom is not available, LPA Lowe advised restroom will need to be cleaned and sanitized before use of any other child. Children’s authorized representative will be contacted immediately for pickup, if a child becomes ill. Department of Public Health will be contacted in regard to any COVID-19 illness.

Children will stay with the same group as assigned and will not be moved from one group to the other. Stable groups should be as small as feasible. The maximum size of stable groups is limited by the number of children that allows all members of the group to maintain physical distancing of 6 feet from all other members within the available licensed space. At no time may group size exceed the maximum number of children permitted by Community Care Licensing (CCL) per classroom or other licensed space, and facility shall comply at all times with minimum staff to child ratios set by CCL.

The Staff Work Area room was observed with a mesh child proof gate at entrance to space, outlet covers were in place, and age appropriate furniture to meet the needs of children in care. Page 1 of 3

The Common Area was observed to have age appropriate furniture. LPA Lowe observed one long rectangular table with 10 chairs in the space. LPA Lowe advised adding additional tables to ensure 6 feet distance between children in attendance in both the common area space and the staff work area space. Both rooms are currently being used by preschool age children.
Rooms designated for school age use are Enrichment Studio and the Adult Work Area. No school age children in attendance at time of visit. The enrichment studio was observed to be an open space with little furniture with the intended use for gross motor activities. The adult works area was observed to have individual desks set up for social distancing for school age children to participate in distance learning.

LPA Lowe observed 2 fire extinguishers located in the hallways. Facility has a sprinkler system.

Current outdoor space is under construction and unable to use by children. With school age not in attendance LPA Lowe observed the preschool children from the common area use the enrichment space. LPA Lowe advised designated school age space can not be used by other groups. LPA Lowe advised due to COVID-19 children have to remain separate in their own space and no crossing into other cohorts.

Children bring their own lunch from home or they may receive a prepackaged lunch from an outside vendor. Children bring their own water bottles from home and the facility will provide water refills.

LPA Lowe observed one bathroom located in the staff work area room with 1 sink and 1 toilet. Hand washing sign was posted and a one step - step stool was available for the children at the sink. LPA Lowe observed another bathroom in hallway near the common area to have 1 sink and 1 toilet, hand washing sign was posted.

The facility will maintain children files that include emergency authorization cards and staff files that include proof of criminal background clearance. All childcare areas, including the playgrounds, will be cleaned and sanitized daily or more often as necessary following the facilities cleaning and sanitation policies.

A health check on staff and children will be conducted each day upon arrival. Owner stated parents are asked screening questions at the time of booking space as well as at drop off each day. LPA Lowe discussed the terms and conditions of the waiver and facility will ensure compliance with all terms and conditions of the Temporary Waiver and will adhere to Public Health guidelines and directives.
