On June 15th, 2021, Licensing Program Analyst Lisa Rios called Director Kenner Rakov and owner Melanie Wolff to discuss some areas of concern that were noted during the investigation into the complaint filed on 3/25/21 (control number 30-CC-20210325164042).

During the course of the investigation, on March 26th, 2021, owner Melanie Wolff submitted to the department the names of 4 staff that were being used as substitute teachers. Adult 1 does not have a criminal background check clearance and proper documentation to transfer Adult 2's criminal background clearance was never submitted (see LIC812 Email Owner-Substitutes dated 3/29/21).

On April 20th, 2021 Licensing Program Analyst Lisa Rios discovered that the facility does not have their facility numbers issued on any of their online advertising (see LIC812 Facility Advertisement dated 4/20/21).

On April 20th, 2021 Director Kenner Rakov sent sent screen shots from April 16th, 2021 of the facility's sign-in sheets to LPA Lisa Rios (see LIC812's dated 4/20/21 and LIC812 interview with S1 dated 4/28/21). They established that the teacher's have been signing the children in at drop off and signing the children out at pick up.

On March 26th, 2021, Licensing Program Analyst received the necessary documentation to associate four people to both facility licenses that were already being used to fill in for absent staff. On April 30th, 2021 LPA Rios requested the personnel files for these staff to qualify them as teachers or teacher aides. Necessary documentation to qualify these personnel was not available. One substitute does not have criminal record clearance and one substitute did not have the necessary documents submitted (LIC9182, copy of driver's license and LIC508) to make the transfer of the criminal record clearance.

On April 30th, 2021, Licensing Program Analyst Lisa Rios emailed Director Kenner Rakov asking for personnel files for the 4 subs used at the facility in April, to determine if they were qualified to work with children. These personnel files were never sent in to the department (see LIC812 dated 4/30/21).

On April 15th and May 2nd, 2021 , Licensing Program Analyst Lisa Rios requested that eight documents be signed and sent back to LPA Rios. These documents were 9099's and 809's from Complaint control # 30-CC-20210302101833 under license # 197494347 and 809 case management deficiencies dated 4/15/21 under facility # 197494347.

On May 5th, 2021, Licensing Program Analyst Lisa Rios requested via email from Director Kenner Rakov paper sign-in/sign-out sheets that the parents sign the children in on at drop off and out on at pick up.
These were never submitted to the LPA Rios.
