On 10/11/19, Licensing Program Analyst (LPA) Karren Starks made an announced visit for the purpose of conducting a pre-licensing inspection. LPA met with Regional Site Directors, Nubia Juarez and Irma Martin along with Compliance Coordinator, Krystal Fluker. Director Kellie Konysky arrived during inspection. The applicant is requesting licensure for 8 toddlers ages 18 months - 3 yrs and 20 Pre-school children ages 3-5 years for a total of 28 children. A granted Fire clearance has been received for the location. The facility will operate from 8:00am - 2:30pm with full day programs.

The facility consists of two rooms that have separate entrances. The Toddlers will enter from the Broadway Street entrance and the Preschoolers will enter from the 7th St. gate. LPA inspected both rooms and observed parent boards with the required postings including a sample menu. There are cubbies for the children's belongings. Age appropriate furniture and toys were observed. In both rooms LPA observed area rugs that need to secured to prevent tripping. Both rooms have the rooms divided into different manipulative stations. All electrical outlets were covered. LPA observed fully charged 2A10BC Fire extinguishers in both rooms, carbon monoxide and smoke detectors as well as First Aid Kits with Instructions and Disaster Kits. An Emergency bag. There is a water dispenser in each room, making water readily available to children. Napping equipment was observed. Sinks for handwashing were observed as well as step stools for assistance in reaching. There is a changing table on the Toddler side within reach of a sink.

The kitchen which is located between the two rooms was inspected and LPA observed a properly functioning refrigerator. The facility will have Food Service provided through CAFP a central kitchen that prepares the meals and delivers them to the sites. An allergy list was posted in the kitchen. LPA tested the temperature of the water in the kitchen and it was not hot enough. A supply of emergency food was observed and no toxins or cleansers were stored with the utensils. The kitchen is made inaccessible by a child safety gate.

The bathroom which is also located between the two rooms, was inspected and LPA observed 3 properly functioning toilets and sinks. There was a sufficient supply of toilet paper, soap and paper towels.

The facility will be providing Incidental Medical Services Plan.

Prior to LPA leaving the kitchen water temperature was corrected and the trash cans with lids were delivered.

The outdoor area was inspected. It is a fully gated area with concrete and grassy areas. The facility is looking to use the Toddler entry side of the yard for the Toddler with a climbing apparatus placed on the cushioning area of the pre-school side. Per Ms. Konysky the are planning on separating the current outdoor space or requesing a waiver for joint usage of the outdoor space. There is a small gated area next to the parking lot that is fully gated that can provide the separations needed for the different components.
LPA observed concrete areas for active play, grassy area that had manipulative house like platy stations. Picnic like table were observed as well as riding toys.

The small play area measured for a total of 10 Toddlers, the large or preschool playground measured for 66 children.

The facility is able to have the requested capacity of 28, 20 preschool 8 toddlers based on the measurements taken.
Prior to licensure the following must be completed or corrected no later than November 10, 2019, if not received the application may be denied:
1) Area rugs must be secured
2) Shade must be obtained for the Toddler play area
3) Incidental Medical Services Plan

substantiated complaint investigations; copy of licensing documents pertaining to a conference conducted by a local licensing agency management representative and the licensee of this family child care home in which issues of noncompliance are discussed or copies of a summary of an accusation indicating the Department's intent to revoke the facility's license. Copies of any of the above licensing documents the licensee has received in the prior 12 months shall be provided to parents/guardians of newly enrolled child at the facility.

Senate Bill (SB) 792: This bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.


Senate Bill (SB) 277 New Immunization Requirement: Beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

Licensee was made aware of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
