On 09/26/2024 LPA Clayton arrived at the CCC unannounced for a Case Management - Other inspection. LPA Clayton was greeted by Shan Silva. LPA Clayton toured the CCC for a Health & Safety Inspection, and observed 13 children being supervised by 2 fingerprint cleared staff.

LPA Clayton advised Shan that several attempts to reach the facility regarding past due licensing fees by phone were made. LPA Clayton advised Shan that the phone number associated with the CCC (310) 815-8314 is disconnected. LPA Clayton instructed Shan to have the owner complete a LIC 200 updating the phone number and mail to the EL Segundo Regional Office no later than Monday September 30, 2024.

LPA Clayton advised Shan that the CCC has past due licensing fees in the amount of $726.00. LPA Clayton provided Shan with the El Segundo Regional Office number to obtain the PIN and instructions on how to pay the past due fees online.

LPA Clayton also advised Shan to submit his Director Qualifications form and transcripts no later than October 4, 2024 for review and approval.

Exit interview conducted and report was reviewed with the Shan Silva. LPA Clayton posted a Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
