On 1/6/2020 at 11:10 A.M. Licensing Program Analyst (LPA) Veronica Wheatley conducted an announced in person prelicensing inspection to Arise Family Child Care Center for the purpose of a new license. The applicant is requesting 30 preschool children ages 3 years old to 5 years old. LPA met with applicant LaRhonda Mobley who provided a tour of the facility. The preschool will operate in 3 rooms. The fourth room is being used as storage at this time and is off-limits. An approved fire clearance is on file conducted by inspector Michael Judkins of the Los Angeles County Fire Department. The facility has cameras in classrooms. The following was observed:

ENTRANCE - Hand sanitizing station due to Covid-19, thermometer, clean pens, face mask, etc.

The required signs are posted in a prominent area for parents to view.

INDOOR ACTIVITY SPACE

Fire extinguishers (two) 2AB10C and carbon monoxide detector were observed First aid kits were observed with the required essentials: scissors, bandages, tweezers, and thermometer.

Age appropriate toys and equipment were observed in good repair. LPA advised that toys made of cloth be removed due to Covid-19 to prevent the spread of germs. The facility is equipped with central heating and air conditioning to control the room temperature. In addition, there is a portable air conditioner in the small room. Adequate lighting was observed. The classrooms are clean and rugs were in good condition. Trash bins used for solid waste were observed with tight fitting petal operated lids.

Disinfectants, cleaning solutions and other toxins were made inaccessible to children, placed a locked storage cabinet.

INDOOR ACTIVITY SPACE CONTINUED
The children will nap in the front classroom. LPA observed age appropriate cots. The sheets and blankets will be kept in a zip lock bag with the child's name and will be washed weekly by the facility staff.

The measurements for the front classroom measured for 17 capacity. The middle room before kitchen measured for 4 capacity and the area near the director's office measured 5 capacity to equal 26 capacity.

RESTROOMS


LPA observed two clean and sanitized restrooms for children with the necessary toiletries. The sinks and toilets are operable and in good repair.Toilets and sinks are age appropriate. Faucets delivered cold water only. LPA observed:

2 toilets = 1 toilet per 15 children = 30 capacity


2 sinks = 1 sink per 15 children = 30 capacity
= 30 capacity/children

CAPACITY BASED ON SINKS = 30 capacity


CAPACITY BASED ON TOILETS = 30 capacity

One staff restroom was observed on the premises.

Food Services Area

LPA observed a food preparation area with a refrigerator, microwave oven, and sink. The hand washing signs are posted. LPA observed hand soap and paper towels. LPA observed several food and bottled water. LPA observed the menu posted.

OUTDOOR ACTIVITY SPACE

Age appropriate toys and equipment were observed in the outdoor activity space in good repair. The outdoor play space is fully gated with a gate at least 4 feet high. The yard has soft cushion under the age appropriate play structure. This will be maintained to prevent tripping hazards and injury. LPA observed a canopy which provides shade and table and chairs for rest. Drinking water will be provided to the children at all times to freely drink. No hazardous conditions or equipment was observed during today’s visit.

Measurements for the outdoor activity space = 1420 divided by 75 SQ. FT per child for a total of 19 capacity/children.

The applicant is requesting a waiver for Title 22, section 101238.2. Outdoor Activity Space that states in part: (a) There shall be at least 75 square feet per child of outdoor activity space based on the total licensed capacity.

Based on measurements and observations the center shall be recommended for licensure pending approval of the requested waiver for a capacity of 26 capacity determined by the facility fire clearance.

A fire clearance has been received by the Los Angeles County Fire Department for 30 capacity.

LPA did not not observe any baby walkers, bouncers or exersaucers during today inspection.

LPA observed a separate classroom with a door which is currently being used as storage. The applicant plans to use the room in the future however the room is currently off-limits.

The director's office will be utilized as an isolation area.

The following items were thoroughly discussed with the applicant:
*Covid-19 Guidelines in regards to check in and screening screening children upon arrival. Taking temperatures, washing hands for 20 seconds or longer with soap and water. Hand sanitizing stations for parents at entrance and available when children cannot was their hands. The hand sanitizer must be at lease 60% alcohol and kept inaccessible to the children in care.
*Requirements for fire drills, earthquake drills, and documentation for both
*The necessity of earthquake safety and drills every month
*The role and responsibilities of being a mandated reporter
*Mandatory Forms for the children’s files. Records must be kept for 3 years after termination.
*LPA Wheatley informed applicant how to obtain Quarterly Updates & PINS (Provider Information Notices). Applicant was encouraged to read the Child Care Quarterly updates and PINS as they come out, to stay informed of any changes or updates to statutes and regulations.
*The applicant was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation on (LIC 624B)
*The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents. *The applicant was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days.
*LPA discussed AB633 and informed applicant that upon receipt of a Type A deficiency, the applicants shall post and provide copies of this licensing report (LIC 809 or LIC 9099 and copy of LIC 9224 parent/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed.
*Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Due to the COVID-19 pandemic and local and state ordinances, applicant was directed to the CCLD.CA.GOV website for important updates and PINS. Applicant was directed to PIN 20-06 regarding social distancing and healthy practices to follow.

The facility is in compliance according to Title 22 Regulations and Covid-19 Guidelines and will be licensed for 26 capacity after the playground waiver is approved by management.

An exit interview was conducted and a copy of the report will be emailed to the applicant with read receipt instructions.
