On 6/13/2021 at 2:35 pm, Licensing Program Analyst (LPA), Deborah Lowe made an announced visit to Destiny Development for the purpose of conducting a case management visit. LPA Lowe met with licensee Danielle Williams and Uchechukwu Chris Nwani, Chief Executive Officer. LPA Lowe toured the facility. The licensee is requesting to reduce preschool and toddler option capacity to 59. Requested hours of operation Monday - Friday 6:30 am - 6:00 pm.

Measurements - all classrooms are square shape.

Toddler classroom is located last classroom on west side of facility.

(26 x 26.75) = 695.50 Measurement total for toddler classroom space: 695.50 divided by 35 sq ft for a total of 19 children.

Preschool classroom west is located between toddler classroom and preschool classroom east. (26.42 x 24.17) = 638.57 Measurement total for preschool classroom space: 638.57 divided by 35 sq ft for a total of 18 children.

Preschool classroom east is located between other preschool classroom and school age classroom. (24.05 x 26.05) = 626.50 - (encumbered space) = 6.15, 620.50 - 6.15 = 620.35 Measurement total for preschool classroom space: 620.35 divided by 35 sq ft for a total of 17 children.

Measurement totals for classroom space: 695.50 + 638.57 + 620.35 = 1954.42 divided by 35 sq ft for a total of 55 children.

Preschool playground (69 x 53.42) = 3685.98 - (encumbered space) 132.25 = 3553.73
Preschool outdoor space is 3553.73 divided by 75 sq ft for a total of 47 children.

Designated toddler climber (15.17 x 13.33) = 202.22
Concrete area with toddler climber area (22.5 x 10.05) = 226.13 toddler area total 202.22 + 226.13 = 428.45
Toddler outdoor space is 428.45 divided by 75 sq ft for a total of 5 toddler age children.

Restrooms:
Children's bathrooms are located in the hallway. Licensee stated a staff members escort children to and from the bathroom to maintaining supervision. The preschool was observed with bathrooms sectioned into three parts. Entrance to bathrooms - section 1 bathroom with 2 toilets, 2 urinals, and two sinks, section 2 was observed with 2 sinks along the wall and a single toilet with a non locked door, section 3 was observed with 1 toilet and 2 sinks.

Bathroom calculations - 6 toilets/urinals = 1 toilet per 15 children for a total of 90 and 6 sinks = 1 sink per 15 children for a total of 90.

Indoor Activity Space:
LPA Lowe observed indoor activity space to have age appropriate furniture and toys in good repair, including tables, and chairs. Cubby storage for the children's personal belongings is located in the classroom. An Igloo water container was observed for water.
Toddler room was observed with changing table, portable sink next to changing table and soft furniture.

Fire extinguisher was observed with a service date of 4/16/2021. Facility is equipped with a fire alarm system. Carbon monoxide detector was tested, LPA Lowe was able to hear the alarm. Smoke detector was observed in the classroom, smoke detector was tested, LPA Lowe was able to hear the alarm. Trash cans were observed with appropriate lids.

First aid kits were observed to have: bandages, adhesive tape, scissors, tweezers, and antiseptic solution. Thermal thermometer was observed at the front entrance of facility.

Directors office will be used for isolation of ill children. Required posting were observed in hallway near the entrance for public viewing. Parent / guardian sign in and out is at front entrance of facility for all children to be signed in with original signature. Facility has a working phone land line available.

Medications will be stored in individual classroom. LPA Lowe observed a locked medicine box bolted to the wall inaccessible to children. Refrigerator is available for refrigerated medications.

Food Service:

Facility participates in the California Food Program. Licensee stated kitchen is not used, snacks and lunch is delivered daily for all children. Refrigerator was observed to be in good repair. Individual wrapped non-perishable foods were observed to be stored in a plastic container, Licensee stated they use those as a back up.

Outdoor Activity Space


Preschool playground is west of entrance door (entrance door is located in back of building away from street).

Age appropriate toys and equipment were observed in the outdoor activity area and in good repair. Water Igloo will be taken outdoors with class to have cold water available to children. Play yard was enclosed by a gate higher than 4 feet. Climber was observed on the preschool playground with cushioned material to absorbs falls. Climber was under shade structure. Poles to shade cover were padded. Shed is located on playground on the northwest corner. Shed was observed to be locked.

Toddler outdoor activity space was observed to have a small size climbing structure with shade and with cushioned material to absorb falls. Toddler climbing structure is sectioned by child gate.

The following was discussed with the applicant:

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Senate Bill (SB) 792: This bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.

Senate Bill (SB) 277 New Immunization Requirement: Beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

SIDS & SHAKEN BABY SYNDROME INFORMATION: LPA discussed safe sleep for infants with applicant: Infants must be placed on their backs and must be physically checked every 15 minutes to gauge temperature and ensure they are breathing. Applicant reviewed both items provided and understands the guidance of safe sleep practices. LPA discussed flyer given today (Never Shake a Baby) Applicant reviewed flyer and understands the preventive practices of shaken baby syndrome and abusive head trauma.

Licensee shall maintain Administrative Records which shall have the following:
Administrative Records
Written inspection procedures for accepting children on a daily basis.
Sign-in/sign-out sheets kept for current 30 days, or approved waiver to use electronic pin system.
Admission policies, including admission criteria, ages of children who will be accepted; medical assessment requirements; program activities, supplemental services, if any; field trip provisions, transportation arrangements, food service, if any.
Designation of Facility Responsibility (LIC 308).
Personnel Report (LIC 500) showing current roster.
Licensee affidavit regarding persons exempt from fingerprint requirements (Use back of LIC 500).
Emergency Disaster Plan (LIC 610) (a posting requirement; see below) with verification that disaster drills are conducted every six months. Documentation of drills shall be maintained for at least one year.Up-to-date list of qualified teacher substitutes.
Documentation of exceptions and waivers: Facility Waiver Request (LIC 956) and Exception/Exemption Request (LIC 971).
Annual licensing reports and substantiated complaints from the last three years (must be available at the center for public review). and a Child Care Facility Roster (LIC 9040).

CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Upon final administrative review and outstanding supporting documentation needed, final decision of capacity decrease License issuance will be determined by the department unit Licensing Program Manager.

During exit interview and review of facility measurements licensee stated they are considering withdraw of the toddler option component. LPA Lowe advised a new application will need to be submitted.

Exit interview was conducted over the phone on 6/14/2021 with Danielle Williams, Owner/ Director.
